Greetings Ma’am!

We are from Grade 12 Humanities and Social Sciences having a research entitled “The Level of
Mathematics Anxiety of Grade 9 Students in Western Leyte College of Ormoc” in the partial
fulfillment of the subject Inquiries Investigation and Immersion.

This study aims to determine the Level of Mathematics Anxiety of Junior High School Students
here in Western Leyte College of Ormoc. Upon conducting the study, researchers will use a
readily made questionnaires. The research instrument will be distributed in hard copy in order for
the respondents to access easily. Conducting the study will be done during vacant classes
schedule to ensure no interruption of discussion to any classes.

Rest assured that the study will done ethical and professionally. The data and information
gathered will be considered confidential and only the researchers are allowed to evaluate the
findings. In addition, the research instrument use for conducting the study is attached along with
this letter.

In line with this, we humbly ask for your permission to conduct our study inside Western Leyte
College campus and allow all Junior High School Students to participate in order for us to
determine their Level of Mathematics Anxiety.
