AFFIDAVIT OF NO INCOME TAX RETURN

I, JEAM BOY VISITACION LAMPAD, of legal age, Filipino, Single and a resident of
#84 Araullo Street Barangay Addition Hills, Mand aluyong City, Manila Philippines after
having been duly sworn to in accordance with the law do hereby depose and say;

That I am a Customs Broker and opened a brokerage registered as an individual (Jeam Boy
V . Lampad)

That my activities involves in processing of shipment entries as required by law in the

Bureau of Customs;

That i am not gainfully employed with any private or public entity, natural or juridical,
thus, I am not filing any Income Tax Return with the Bureau of Internal Revenue;

That I am executing this affidavit to attest the above stated facts and as a requirement in
the accreditation of my business and for whatever legal purpose this may serve.
