I respectfully request for consideration regarding the unavailability of the copies of my Service
Record, Performance Rating, and School Clearance because I haven’t had a teaching experience yet, but I
believe that I am ready to take the responsibilities of a teacher as I have already equipped myself with
skills and learnings which I have gained through the series of lectures I attended in my college years and
during my internship in Demetrio L. Alviola National High School.

I would also like to request for consideration regarding the unavailability of my PRC License for
I wasn’t able to attend the Mass Oath Taking Ceremony that was supposed to be on the first week of
February, 2020 due to some health complications. For now, I am patiently waiting and praying for the
schedule of online oath-taking application system (e-oath) and I was informed that they’re going to
conduct it this month of February. I deeply apologize for this inconvenience for I cannot allow this
opportunity to pass away without even trying. My dream is to be part of the DepEd family, if God-
willing, then I would be able to fulfill my goal which is to educate young minds, not just textually, but
also in the spiritual sense.
