AFFIDAVIT OF CONSENT

I, ANGELICA BUIZON-DEERING, of legal age, married, residing at Blk 25

Lot 9, Carnation St., Merry Homes Subd., Zabarte Road, Camarin, Caloocan,
Metro Manila, after being sworn to in accordance with law, depose and say:

1. That I am the rightful owner of a parcel of lot, located in Zone 1,

Brgy.. Basagan, Tabaco City, Albay ( a copy of Deed of Absolute Sale of a
Portion of Real Property is attached, marked as ANNEX A an made an
integral part hereon);

2. That on the aforesaid parcel of lot, (lot No. 1571-D), there is a one-
story residential building erected thereon;

3. Currently, the occupant is my mother SHIRLEY TUSCANO and my

sibling ROSELYN T. BUIZON;

4. That I am giving my consent/authorization to my mother and

sibling, to utilize the said property as their dwelling;

5. Likewise, I am giving my consent for them or their duly authorized

representative to submit an application for the installation of electrical
services from ALECO on the subject property;

6. That I am executing this affidavit for the above-mentioned purpose

and whatever legal purpose this affidavit may serve.

2023, in the City of Tabaco, Albay, Philippines.
