Appointment con!rmation

below.

We have emailed a copy of the con!rmation to your


email address below. You may also wish to print a copy
of this con!rmation for your information.

Please ensure you bring all the information and


documents that are required to the appointment.

Health Warning
Please do not attend your appointment if you, your
children or anyone in your household have symptoms
of coronavirus, have been in contact with someone
who has coronavirus or are self-isolating.

What safety measure will be in place?


If you arrive early for your appointment, please wait in
your car / outside the building until the time of your
appointment, as waiting rooms will not be available.

Please adhere to 2m social distancing and enter the


building 1 person at a time. When entering the
building, you must wear a face covering and sanitise
your hands and then report to Reception. You will be
directed to the registration room for your
appointment.

We will be operating a one-way system in the Register


O"ce, where possible.

Safety screens have been installed at Reception areas


and on Registration desks. Surfaces and touch points
will be wiped clean between appointments.

O"ce doors will remain open to allow for air #ow.

Upon completion of the registration, you will be asked


to exit the building directly

Information for your appointment


Our o"ces have limited parking and you will need to
allow time to park and check in for your appointment
on time. Use the link below to check directions to our
register o"ces.

The appointment will be for a minimum of 20 minutes


per child. If you are late for your appointment, you will
not be seen, as there will not be enough time to
complete the registration and you will need to make
another appointment.

If you need to change your appointment, or you are


unable to attend, please call us to reschedule on 0300
200 1002, giving at least 2 working days’ notice.

Who can register the birth?


If the parents were married to each other, or in a civil
partnership with each other at the time of the birth,
only one parent must attend the registration
appointment. Either the mother, father or second
parent can register the birth.

If the parents were not married to each other or in a


civil partnership with each other at the time of the
birth, the father or second parent must attend the
appointment with the mother if their details are to be
included on the registration; if the mother attends
alone, no father or second parent details will be
recorded.

Please note that it is not possible for two male parents


to be recorded on the birth registration. In the case of
two male parents, please call us for advice on 0300 200
1002.

If your child needs to be registered with a Statutory


declaration or a Parental order, please call 0300 200
1002 and ask for the Nominated o"cer at least 2 days
before your appointment for further advice.

How much does it cost?


There is no charge for the registration.

Birth Certi!cates cost £11 per certi!cate and you can


pay at the appointment by debit or credit card. We are
not able to accept cash. The certi!cate is required for
many purposes, such as passport and child bene!t
applications.

Correction charges after registration

You will be asked to check the information recorded by


the Registrar very carefully and then sign the register
to con!rm that it is correct. This is a legal document
therefore it is important that the registration is
accurate. Any correction of errors discovered after the
register has been signed will incur a charge of £75 to
£90 as well as the cost of replacement certi!cates.

The registrar will need to know:


Place and date of the birth of the child
Sex of the child
Chosen name and surname of the child
Parents'' names, surnames and address (including
any former names)
Places and dates of parents'' birth
Date of parents'' marriage or civil partnership (if
applicable)
Parents'' occupations
Mother''s maiden surname (if applicable)

What you need to bring:


Please bring a black pen with you to the appointment
to sign the register page.

You should bring any birth paperwork issued to you by


the hospital or midwife.

To ensure the accuracy of your information, it would


also be helpful for you to bring the following
documents for parents (not essential):

Your passport or birth certi!cate


Proof of your current address
Proof of any former name or names (for example,
change of name deed or marriage or civil
partnership certi!cate )
Birth certi!cate of any previous children
