Description of setting: The student is diagnosed with Major Depressive Disorder on her medical
certificate which is dated at March 2022. In regards to this matter, the school principal, Ms. Obra
advised the student to attend therapy and focus on helping herself get better. She would be only
allowed to go back to school once her recommendation from the doctor is updated, stating that she is
fit to attend face to face classes, rather than her current recorded one on the medical certificate which
states that she is recommended for online classes.

Description of incident: The recent panic attack in the bathroom of the third floor Junior High School
building of Ms. Munar caused a panic to some students who were also present there. She was
rushed to the clinic with the aid of the other students and her adviser. Afterwards, she was advised to
calm down and see Ms. Obra for further consultation and insight regarding her mental health
situation. According to her medical certificate that is dated at March 2022, she is recommended to
have an online class instead of a face to face, and with this reason and evidence, Ms. Obra
suggested her of getting a new medical certificate. A scheduled talk with her guardian and adviser
happened at October 28, 2022. Within the conversation, both parties agreed to let Ms. Munar take a
break and focus on attending check-ups and therapy, helping her record be updated while helping
herself overcome that state.
