Madam:

This has reference to my earlier approved two months vacation leave without pay which will end
on May 31, 2019 for foreign trip to the U.S.A.. The untimely emergency my daughter , WYNTEE
MAY VELASCO-CRUZ, Teacher III of the same School encountered and afflicted with “Acute
Myeloid Leukemia” (AML) as diagnosed by Doctors after several series of laboratory
examinations. For this reason, I would like to seek once again your utmost understanding and
consideration to please extend my leave of absence for another Sixty One Days starting on June
1, 2019 to July 31, 2019. The extension will be on sick leave basis since my daughter is sick.

Presently, WYNTEE is now battling the doctor’s diagnosis for LUEKEMIA at Baguio General
Hospital and Medical Center, Baguio City after we transferred her from Notre Dame de Charters
Hospital, Baguio City and now undergoing a series of chemotherapy treatment.

As a father and head of the family, I am duty bound to give my utmost support to my loving
daughter who is in need of my attention especially in this critical stage of her therapy. My sick
leave of absence is to be physically available at her bedside. Much to my desire to supposedly
report back for work, but my duty and obligation to attend to the needs of my daughter is of prime
concern.

Your favorable consideration on my request is highly appreciated as I pray to the Almighty to

enlighten your motherly heart to grant the above stated request. Thank you so much.
