April 3 and 4, 2024 which will be held at Bayombong Central School, Bayombong Nueva Vizcaya. In line with this

WAIVER OF RIGHTS AND CONSENT FORM

TO WHOM IT MAY CONCERN:

In connection with the conduct of the Division Schools Press Conference on April 3 and 4, 2024 which will be
held at Bayombong Central School, Bayombong Nueva Vizcaya and considering the benefits my son/daughter/ward
can get from the said activity, we do freely express our permission to allow our son/daughter/ward to join and
participate to the Division Schools Press Conference (DSPC).

Further, we do hereby waive our rights to execute any legal actions and constitute any claim against and from
the school administrators, teachers and staff of Dupax del Sur National High School and we do not hold them liable
for any untoward incidents that may happen during the conduct of the said activity which is beyond their control
provided that due care and precaution will be observed to ensure the comfort and safety of my son/daughter/ward and
that DepEd and DDSNHS employees and personnel may not be held responsible for any untoward incident that may
happen.

1. The Department of Education – SDO Nueva Vizcaya, through the Curriculum Implementation Division
(CID) will conduct the 2024 Division Festival of Talents on April 11-12, 2024 with the theme “Galing,
Talino at Husay ng mga Batang Makabansa sa Diwang MATATAG na Adhika” in which Dupax del Sur
National High School will host the Division Festival of Talents for Filipino and MAPEH (Elementary and
Secondary) on April 12, 2024 (Friday) .
