PROCEEDINGS
The virtual LAC Meeting was officially started at 8:11 AM and it was facilitated by
Mrs. Jessica Ruiz during the conduct of the preliminary parts of the meeting,
presentation, and discussion of the agenda via Zoom Meeting. The emphases of the
meeting were to deliberate on the forthcoming celebration of the 12th Foundation
Anniversary and Christmas Program which will be hosted by the TLE and MAPEH
Groups. Also, to tackle the concerns on the Quarter 1 Assessments, and Card Giving
preparations.
Before the discussion of each agendum, Mr. Jason Pascaden gave reminders
concerning the submission of LAC report/s which will be submitted 3 days after the
conduct of the LAC meeting. There were some grade level hosts of the previous LAC
meetings who did not submit the after-the-meeting documentations or reports yet.
Likewise, the previous LAC meeting by learning areas must submit the after-the-
meeting reports on the update of the Learning Management System and the like.
After the giving of reminders, the meeting proceeded on the presentation and
discussion of the first agendum which was presented by Ms. Jonalyn Belting, the
Chairman of the 12th Foundation Anniversary and Christmas Program of the school. The
celebration of the said programs will be on December 15-16, 2021 respectively. The
details of the program were summarized below:
12th Foundation Anniversary

Zumba Dance
Vermiculture Livelihood Program
Remarks:
- There should be 1 parent representative per section.
- Selected parent representatives will participate first in the Zumba Dance.
(not interested to join the Zumba dance may watch the dance session)
- The best Zumba dance performer among the parent representatives will be
selected on December 15, 2021.
- After the Zumba dance activity, Vermiculture Workshop will follow.
- Each parent representative will receive a vermicompost kit worth of 200
pesos.
B. Venue: Use the School Grounds for the Zumba Dance and Vermiculture Program
or utilize some of the classrooms.
C. Theme
Remark: Everybody is encouraged to submit his/her entry on the formulation of the
12th Foundation Anniversary best theme on November 22, 2021

The second agendum was on the Regional Memo. No. 545 s. 2021 or the
Application for Safety Seal Certification by All Schools Division Offices in the Region.
Ms. Lucia Casim, the schoolhead gave a brief presentation regarding this Regional
Memo. According to the Memo., the Department of Education Cordillera Administrative
Region (DepEd CAR) has been consistent in implementing programs, projects, and
strategies to take part in the realization of DepEd Vision and Mission. Moreover, amidst
the Covid-19 pandemic, maximum efforts and resources were exerted and utilized to
ensure safety of its employees and clients by adhering to health protocols and
guidelines, hence its recent Safety Seal Certification issued by the DILG La Trinidad
Municipal Office and PNP.
The DepEd CAR directs all Schools Division Offices (SDOs) and schools to
apply for Safety Seal Certification from the office of the DILG to serve as a challenge
and inspiration to continue ensuring safety in the workplace. The following are the
content of the Safety Seal Certification Checklist: Contact Tracing Tool, Availability of
Thermal Scanner, Health Declaration Form, Isolation Area for Symptomatic employees,
BHERTs and other Covid-19 hotlines, Availability of Handwashing Areas, Installed
Physical Barriers in enclosed areas, Availability of Personnel- in-charge for monitoring
and maintaining social distancing, Availability of windows for adequate air exchange in
enclosed areas, Compliance the disinfection protocol, Wearing of facemask and face

Before the presentation of the LAC Agenda, a brief review of the previous LAC
Meetings was made by Ms. Honda Freda Sabado.
Ms. Jessica Ruiz, the LAC Facilitator marks the start of in-depth discussion of the LAC
the first Agendum.
“Refining of the 12th Foundation Anniversary Program is welcome for the best”,
reiterates by Ms. Lucia T. Casim.
The school needs to assess its preparedness and sustenance in terms of the
availability of equipment and resources for the Application of Safety Seal Certification.
