Title of Activity: Conduct of Values Formation

Objectives:
1. To address community needs and expectations that are focused on
education, values formation and basic social services.
2. To mold the youth to become responsible members of the society.
3. To increase awareness of the importance of moral, spiritual and social
values.

Expected Number of Participants: 20 Grade 6 Learners

Actual Number of Participants: 20 Grade 6 Learners

PART II

NARRATIVE REPORT

1. Introduction/ Rationale
Pursuant to the DepEd MATATAG: Bansang Makabata, Batang
Makabansa," one of the salient points is to intensify the values formation of
learners in the curriculum. It is observed that school learners are being
influenced by many factors that really affect change of behavior and values.
In addition, students need also to be reminded of their spiritual roles as they
continue to journey to become effective individuals bringing the core values
of the Department as MakaDiyos, Maka-Tao, Maka-Kalikasan at Maka-
Bansa. This activity is also a way to intensify the advocacy of being a Child-
Friendly School and Bawat Bata Butihin.
2. Narration of Accomplishment in accordance with the Training/ Activity
Matrix/Schedule
June 29, 2023, a day wherein chances are given to learners to
realize things, to contemplate and to meditate. This is the right time to open
their minds for acceptance and repentance and that they need to face the truth
and the realities in life.

The activity started at exactly 8:00 in the morning facilitated by

Ms. Jane Paula D. Dapidap, one of the member of the United Pentecostal
Church. The event started with a prayer song led by Mrs. Mary Joy M. Dacia
followed by opening remarks of the School Head,Sir Antonio R. Datuin. Bible
stories sharing was given by Ms. Mary Joy D. Canduli and Mr. Lance Florence
A.. Anido. The Grade 6 learners attentively listened to the story tellers because
everyone was instructed to share their own experiences after the activity. A
Cooperative Game was done to motivate the learners.Pastor Richard A. Dacia
then introduced himself and gained the trust and interest of the learners.They
listened as he delivered his thoughts. The day was full of
enlightenment.Learners showed different emotions; happiness, sadness, regrets
and fears. But everyone was hopeful for the next days of their lives.The activity
went smoothly,Everyone shared their experiences,realized and accepted their
shortcomings as a person with differences.The activity ended with giving thanks
to their teachers and a heartfelt prayer led by Pastor Dacia.
Our values inform our thoughts, words and actions,they help us
to grow and develop and they help us to create the future we want to
experiences. Everyone was hopeful that the aim of this activity was achieved
and will bring changes to the bahaiors of our learners
