Subject: MOU For Affiliation for NSDC

Dear Sir,

Based on our recent discussion, we are pleased to engage our services as

“Consultant” as per following terms and conditions

1. Period of Contract: This agreement made effective from 24th April 2023
for period of one year. However, the agreement remains valid till
1st April 2024 gets NSDC affiliation.

for getting NSDC TP Affiliation. This work involves helping to prepare the
complete NSDC Training Partner proposal, Submission, follow-up with NSDC
official (for TP Team) and ensuring the required changes from time to time as
per NSDC request.

4. Documents to be submitted:
A. Registration Certificate
B. Last 3-year AUDIT, ITR and annual report
C. IT PAN Card
D. Trust or Company Deed
E. Board of Directors’ detail (brief CV)
F. Proposed location for Assessment
G. Preferred Sectors for Assessment
 H. Preferred Courses of Assessment

5. Terms of Contract - Remuneration: For the above services, we will

charge Rs. 3, 00,000/ (Three lakh Rupees Only).

6. The payments will be made in following stages:

a. 66 % amount i.e. Rs. 2,00,000 (Rs. Two Lakhs Rupees) as an Advance
on signing MOU.
b. Balance 34 % i.e. Rs. 1,00,000/- (Rs. One lakh rupees) after getting
affiliation.

Expenses: All the travel expenses incurred by you for this assignment will be
reimbursed by Kaila Devi Health Care Services Private Limited .

7. Termination of Contract: The contract could be terminated mutually by

either party giving thirty days’ notice in writing.

8. Acceptance: You are requested to confirm the acceptance of the terms &
conditions set forth in this agreement by signing on the duplicate copy of
this and returning same to the undersigned.
