Description of the Incident:

On June 7, 2023, at 3:30 PM on a Wednesday at San Francisco, Iriga City, Mr. Gordo
went to home from school early; However, upon entering at their house he saw a guy poured a
gas right side of their house, he saw as well how the guy lighten up a lighter then put straight
where the guy poured the gas. He recognized the guy who arson their house but Gordo chose
to help first his family inside the house to evacuate then immediately ask help from the BFP.
After the fire burnt out, the Bureau of Fire Protection (BFP) identified that it is an
intentional fire as they saw evidence that is a bottle of gas and a lighter.

1. DESCRIPTION OF THE INCIDENT:


On June 7, 2023, at 3:30 PM on a Wednesday at San Francisco, Iriga City, Mr. Gordo
went to home from school early; However, upon entering at their house he saw a guy poured a
gas right side of their house, he saw as well how the guy lighten up a lighter then put straight
where the guy poured the gas. He recognized the guy who arson their house but Gordo chose
to help first his family inside the house to evacuate then immediately ask help from the BFP.

2. ACTION TAKEN
Mr. Gordo helped his family to evacuate the house immediately and tried to poured
water to slow the burning fire. After the fire burnt out, the Bureau of Fire Protection (BFP)
identified that it is an intentional fire as they saw evidence that is a bottle of gas and a lighter.

On June 7, 2023, at 3:30 PM on a Wednesday at San Francisco, Iriga City, Mr. Gordo
went to home from school early; However, upon entering at their house he saw a guy poured a

where the guy poured the gas. He recognized the guy who arson their house but Gordo chose
to help first his family inside the house to evacuate then immediately ask help from the BFP.

2. ACTION TAKEN

Mr. Gordo helped his family to evacuate the house immediately and tried to poured
water to slow the burning fire. After the fire burnt out, the Bureau of Fire Protection (BFP)

3. PROGRESS:
The moment Mr. Gordo contacted the BFP they acted right away. They sent out their
crew right away, and they started fighting the fire to put it out and control it. The property had
already sustained substantial harm, though, with the table, the furnishings in its immediate
vicinity, and other items all suffering severe harm. Furthermore, the damaged area experienced
secondary harm as smoke and soot spread throughout it.

DESCRIPTION OF THE INCIDENT:

1. On or about 4:00 PM, this station received a telephone call from Mr. Gordo and
reported that their house at San Francisco, Iriga City is on fire.
2. BFP instantly jiangte Fire Truck that driven by FO2 Gilert San Juan with duty
personnel on board dispatched to respond to said alarm. Immediately
thereafter, Hyundai Fire truck driven by SF03 JOSHUA DAGS with the remaining
duty personnel and off duty personnel followed.
3. General Papay and Real Fire Station also responded to hand some assistance
together with the PNP personnel in Iriga City and as well as the by standers.
4. After several minutes of the Fire Suppression operation, Fire was declared under
control and subsequently declared FIRE OUT at 5 PM of the same date.
5. As the investigator interview the witnesses and the initial conducted by SFO3
Joshua Dags revealed that the fire started outside their house at San Francisco,
Iriga City. Mr. Gordo saw a guy poured a gas right side of their house, he saw as
well how the guy lighten up a lighter then put straight where the guy poured the
gas. He recognized the guy who arson their house but Gordo chose to help first
his family inside the house to evacuate.

4. SUMMARY
It was being used as a backup fire source as a result of the arson as intentional fire. BFP
investigated and collected some evidence that caused the fire which are the bottle of
gas and lighter. Victim mentioned that he saw the guy who poured the gas unto their
house and as the fire started the arsonist ran. Victim stated that the arsonist was their
rival at the market as they sell fresh vegetables; arsonist’s name was EDGAR
BARDAGULAN.

5. Recommendations:
A. Check Outlets Before Use
Behind every outlet is a box with attached wires. And some of the major causes of a
short circuit are faulty wiring, loose box connections, and an aging outlet. Although diagnosing
these issues can be difficult, considering they’re hidden behind your walls, you can still help
prevent short circuits by examining your outlets before each use. There are certain signs that
indicate your outlet may be at risk of a short circuit:
 Outlet has burn marks or burning smell.
 Buzzing or popping sound coming from outlet.
 Sparks emitting from outlet.
 Outlet is over 15-25 years old.
If any of these signs apply, refrain from using the outlet and contact Roman Electric
immediately.

B. Check Appliances Before Use


Just like outlets, you’ll want to check your appliances before plugging them in as well.
Short circuits can also be caused by faulty wiring or circuitry from the appliance itself. Before
every use, check appliances for these signs:
 Damaged cords, casing, or wire.
 Several cracks in the appliance.
 Appliance has exposed circuitry.
If any of these signs apply, we recommend either disposing of or having a professional
repair your appliance.

C. Reduce Electrical Usage During Storms


One of the most dangerous ways a short circuit can happen is by a lightning strike, as
the overwhelming amount of electricity can lead to some serious damage. We recommend
reducing your electrical usage during a lighting storm to only the necessities. Not only will this
help prevent short circuits from happening during a storm – this also helps lessen the damage if
a power surge occurs.

SWORN STATEMENT

1. I, Dave Gordo, of legal age, single, Filipino, a resident of San Francisco, Iriga City,
hereby make the following sworn statement regarding the fire incident occurred
on June 7, 2023 at San Francisco, Iriga City.
2. On June 7, 2023 at approximate time of 3:30 PM I’m coming home from school
early.
3. Before I went inside the house, I saw a guy poured a gas right side of their house,
he saw as well how the guy lighten up a lighter then put straight where the guy
poured the gas.
4. I want to chase the guy; However, I’d chose to help my family to evacuate the
house as the fire is getting bigger instantly.
5. As we evacuated the house, I tried pouring water unto the fire; However, it’s no
use as the fire went bigger because of the gas.
6. I immediately call BFP to help us.
7. The BFP responded promptly to my distress call and arrived at the scene shortly
after. Their team initiated firefighting operations to contain and extinguish the
fire, successfully preventing its spread to neighboring properties.
8. Regrettably, despite the efforts of the BFP, significant damage had already
occurred to the property. The table, surrounding furniture, and other belongings
suffered extensive damage due to the fire and the subsequent smoke and soot.
9. I am fully aware of the importance of providing accurate and truthful
information in this sworn statement. I solemnly declare that the facts stated
herein are true and correct to the best of my knowledge and belief.

AFFIDAVIT OF PHOTOGRAPHER

1. I, Jun Jun Banig, legal of age, residing at San Isidro, Iriga City and a
member of BFP in Iriga City, do solemnly affirm and declare that;
2. On June 7, 2023 at approximate time of 3:30 PM. I was part of the team
from BFP that responded to fire incident at San Francisco, Iriga City.
3. The incident occurred at Zone 4 of San Francisco, Iriga City.
4. That I was the designated PHOTOGRAPHER of the said incident on the
said location.
5. Upon arrival, we saw the family outside trying to poured water unto the
fire. As we hop off the vehicle, I saw already a bottle and a lighter.
6. That I have used OPPO Reno 8 cellular phone with camera in the
photographs;
7. That I took photographs during and final briefing of our designated team
leader for the conduct search pattern against fire incident;
8. As a member of the BFP organization that responded to the incident, I
performed my duties as a photographer and documented the scene
through a series of photographs;
9. The photos I captured during the response accurately depict the
sequence of events as a described above, including the first fire.
10. The images attached hereto are genuine and authentic representations of
the incident that occurred at Zone 4 of San Francisco on June 7, 2023
11. I affirm that this affidavit is made in good faith, based on my personal
knowledge and observations as a member of the BFP in Iriga City.
