INTRODUCTION
Anorexia nervosa is an illness with a high level of diversity because it includes physical
and psychological elements, both of which contribute to the extreme disability and
high mortality associated with the condition.1 Anorexia nervosa often has an enduring
course as illustrated by a recent study, which followed clinical cases presenting for
treatment in the United States. At 9 years’ follow-up, only 31% had recovered,
although by 22 years follow-up, 63% had recovered.2 When adopting a personalized
approach to the management for anorexia nervosa, important questions for consider-
ation are as follows:
What is the level of risk?
What is the form of the illness?

What potential resources from the individual’s social structure and support
network are available?
Clarifying answers to these questions will inform clinical judgment and decision
making. Using knowledge from clinical trials and clinical experience as well as patient
preferences,3 the authors aim to develop an integrated personalized treatment plan
that is both patient centered and evidence based.
Many factors can contribute to medical morbidity in anorexia nervosa, and estab-
lishing the level of risk to the patient is of paramount importance. Body mass index
is a surrogate marker of risk and has now been introduced in classifying the
severity of anorexia nervosa in the Diagnostic and Statistical Manual of Mental Dis-
orders (fifth edition).4 However, its utility is limited, and a multisystem evaluation of
functioning can lead to a more precise risk assessment. In addition, physical health
often interacts with aspects of psychological health, such as potential for self-
harm, alcohol/substance abuse, binge eating, and purging behaviors. A thorough
assessment of a broad range of risk factors should be considered in relation to el-
ements such as motivation, resources, and social support. Higher-risk patients are
often managed in hospital settings with inpatient or day-patient or partial hospital-
ization treatment programs, with some cases requiring treatment on an involuntary
basis.
PREDISPOSING FACTORS
Table 1 summarizes some of the common predisposing factors seen in the develop-
ment of anorexia nervosa. These studies must be interpreted in the context of low inci-
dence rates of the illness,40 because it limits the power of longitudinal studies in fully
identifying significant predisposing factors. One way to circumvent this difficulty is to
examine disordered eating, instead of full syndromal case status. In addition, larger
studies of cases often require retrospective reporting, introducing potential recall
bias. For the most part, where available, Table 1 includes findings from case register
studies, systematic reviews, or meta-analyses.

PRECIPITATING FACTORS
The most common pathway to the development of anorexia nervosa involves dieting
and/or weight loss.41 For most individuals, there is a deliberate strategy to lose weight
or adopt “healthy” eating, although occasionally underlying medical illness may be the
initial precipitant of disordered eating. In addition, reduction in caloric intake may
occur in the context of life events and stressors.
PERPETUATING FACTORS
If anorexia nervosa remains untreated, its clinical signature becomes more severe over
time. Therefore, it is essential that the understanding of perpetuating factors is
improved, because they contribute to a protracted clinical course. In addition, these
perpetuating factors can potentially be moderated and thus are useful targets of treat-
ment. Several of the common perpetuating factors are summarized in Table 2 and
include neuroadaptation, neuroprogression, and social exclusion.42,43 Neuroadapta-
tion is a form of learning whereby addictive or avoidance behaviors become habitual
(see Joanna E. Steinglass and colleagues’ article, “Cognitive Neuroscience of Eating
Disorders,” in this issue). Prolonged starvation contributes to neuroprogressive defi-
cits in brain structure and function.
Social cognition is another domain of vulnerability.39 The reduced expression and
interpretation of facial emotion during the acute phase of the illness contribute to inter-
personal difficulties, which can in turn, impair vocational functioning. Moreover,
people with anorexia nervosa have an increased sensitivity to threat44 and negatively
interpret social scenarios.45 These clinical elements may be consequences of the
state of high stress and arousal from the starvation state. Together, they are apt to
impair social communication and reciprocity.

“isolation” was one word that could encapsulate her experience.46 The impairments in
social cognition outlined above also contribute to the profound sense of isolation
experienced by many patients with anorexia nervosa.
In addition, eating with others is a key element of social functioning, and taken
together with the visible nature of the disorder, anorexia nervosa can have strong re-
percussions on close others. The starved state can elicit strong emotions of terror,
horror, and even disgust in others, which is often combined with a protective
response. Moreover, the resistance to the obvious simple solution “to eat” elicits frus-
tration and hostility. These high levels of “expressed emotion” can serve to maintain or
perpetuate the disorder.47 Extremes of overprotection and hostility can also further
perpetuate avoidance behavior, and families may gradually get drawn into accommo-
dating or enabling the illness. These reactions also may occur with various staff mem-
bers on inpatient/day-care units. A potential consequence of these various emotional
reactions can be the developments of “splits” within the team and family. These fis-
sures can lead to perpetuation of the disorder but also can cause relationship ruptures
and dissipate the support structure.

PLANNING TREATMENT
For the most part, people presenting to child and adolescent services are in the early
stage of the illness, commonly with onset of 1 year or less. In contrast, people in later
stages of illness are more likely to present to an adult service where many have had the
illness for years, and some with recurring episodes. The type of treatment offered,
such as the focus on the family in child and adolescent services, and the expected
outcome, such as poorer response in the context of a prolonged illness in adults, is
different between the services. However, regardless of context, a key for successful
treatment lies in targeting starvation and social exclusion.
TARGETING STARVATION
Weight loss is a key perpetuating factor in anorexia nervosa, because the impact of
starvation on both brain and body lead to consequences that serve in the longer
term to exacerbate rather than resolve the stress and predisposing factors that may
have triggered the illness. There are, however, many uncertainties about when,
how, where, and with what help, weight can be restored and once restored, sustained.
An established and evidenced-based first-line treatment of adolescents is family-
based treatment. Family-based treatment involves empowering parents to feed their

children. The next level may include inpatient treatment, which is an effective method
of restoring weight, but it can be followed by early relapse. In adolescents, a less-
intensive, stepped-care approach with shorter inpatient stays and either day-care or
task sharing with families appear to be a more cost-effective option.48,49
In adults, various forms of psychotherapy, including behavior change strategies to
increase oral intake, have been developed and tested. They all produce similar effects:
the proportion that recovers is small and weight restoration is poorly sustained.50 To
initiate and maintain weight restoration, fear of weight gain and the increasingly con-
trolling and critical “anorexic voice” must be overcome. A personalized approach is
essential to select the most appropriate treatment of an individual.
Promising, targeted approaches are also in development, for example, use of expo-
sure to normal eating and challenging expectations to confront fears while learning
new patterns of behavior by using habit-focused interventions are described else-
where in this issue (see Joanna E. Steinglass and colleagues’ article, “Cognitive
Neuroscience of Eating Disorders,” in this issue). It is possible that new neuromodu-
lation and pharmacologic techniques may have a role in reversing some of the
cascade of problems, such as reducing depressive symptoms, which can develop
during the protracted course of starvation in anorexia nervosa.51
Training carers with the skills to sidestep nonproductive interactions with patients is
important, because it reduces carer distress and enhances patient well-being.
Providing carers of inpatients with self-management tools can reduce carers’
expressed emotion and distress,52 therefore avoiding potential burnout. It also re-
duces readmission in the 6 months after discharge, whereas small improvements in
both carers’ behaviors and patients’ symptoms were sustained 2 years later.53 A
similar pattern of improvements has also been noted by delivering this intervention
to carers of adolescents receiving psychological therapy at outpatient units.54
SUMMARY
In conclusion, our field has come a long way from using treatments that are agnostic
about cause. Newer developments in neuropsychology have allowed us to develop a
better understanding of anorexia nervosa, and we are getting closer to adopting a
personalized approach to treatment. Given the protracted course, any effort in
reducing the devastating impact of the illness could improve the lives of people with
anorexia nervosa and their families.
