1. You must submit this form 6 weeks before the start of new semester.
2. Student is required to attend a consultation session with Programme Coordinator or Head of Academic before they will be allowed
to proceed with the registration.
3. Please bring your results slip for subject(s) confirmation during consultation.
4. A charge of RM750.00 per credit unit will be levied for each repeat subject. E.g: for a 3 credit units subject, the retaking fee is: RM750.00
X 3 credit units = RM 2,250.00.
5. Repeat fees must be paid upon registration.
6. The additional subject(s) as requested will be over and above your current schedule. You must therefore adhere to the requirements of
the added subject(s) with due diligence.
