I. Identifying Information
Client is appealing for financial assistance from the Department of Social Welfare and
Development Field Office VI, Molo, Iloilo City for Rene’s Haemodialysis.
IV. Findings
Rene was diagnosed with CKD Stage V Secondary to Diabetic Nephropathy. He was
advised to undergo haemodialysis treatment for ninety six sessions, under the
supervision of Dr. Joy Gulmatic at Center for Renal Replacement Therapy Inc.
The cost of his haemodialysis treatment per session is P2, 600.00, a total of P 20,800.00
for eight sessions. The total estimated amount for ninety six sessions is P249, 600.00,
which is beyond the capacity of the family to pay due to financial hardships.
Rene is on medication maintenance of the following: NaHCO3, FeSO4 + FA,
Erythropoietin, Losartan, Atorvastatin, Amlodipine, and Eposino.
The family’s financial resources are empty and drained. Furthermore, the family spent a
big amount for Rene’s hospitalization and medications, maintenance that needs to be
prioritized. They owed debt from close relatives and friends which they need to pay as
soon as they have the amount.
Both Rene and Susan are jobless and only dependent on Ryan Sison who works as an
encoder earning P10, 000.00 per month. The need for haemodialysis treatment of Rene
should be prioritize for his well-being.
Family’s income is only enough for their daily basic needs. Hence, they appeal for office
intervention.
Therefore, the undersigned worker recommends the client’s request from the
Department of Social Welfare and Development Field Office VI, Molo, Iloilo City be
granted.
