1. Location
This methods statement covers the procedure of preload removal works of 450 cm for
ST 7.3 & ST 7.4 – STA 112+360-112+810 (Figure 1). This procedure is a working instruction
for PT MITRA GUNUNG MAKMUR personnel and to provide sufficient understanding of all
involved procedure, material, equipment and tools that related to the works. Any deviation
works shall be discussed and coordinated with all involved personnel.
It is proposed that the preload removal part of the runway and shoulder as indicated in the
attached diagram and chainages. This work method including removal, fill operation, spreading
and compacting.
Preload removal start from remove instrument settlement plate no SP1, SP2, SP8, SP9,
SP10, SP11, SP12, SP14, and SP15, the boundaries of the preload removal area on the runway
and shoulders are as described in the figure below. (Fig. 1).

2. Hauling Road
The hauling route by the dump truck from removal preload to the work site is shown
in Figure 2. The haul road must have several conditions that must be met, including:
1. Comfort level
2. Have a high level of safety and security
3. Width enough to pass the transport
4. The haul road profile meets the requirements
5. Road grade is eligible
6. Has a carrying capacity according to the intensity of the passing carrier

7. Have a good drainage system


8. Have a reliable maintenance system for hauling road access
9. Flag man are ready at the location of the hauling road access location
10. Installation of lighting along the hauling road access area
11. Dump Men are ready at the loadng and unloading activity location
12. All dump truck and operational vehicles are required to install rotary lights

3. Material
Refers to “Spesifikasi Teknik Pekerjaan Fasilitas Sisi Udara” page 70, PT MGM use
base course aggregate consists of crushed stone. Base course aggregate produce from crushing
and screening plants with the right mixture according to design mix formula from independent
laboratory.
Aggregate from these sources must meet the gradations requirements, the quality and
consistency of these materials must:
1. Free of vegetative ingredients, excessive clay, and other substances that can decrease the
strength performance and resistance of the base course layer.
2. Mixed uniformly.
3. Able to compacted into a solid and stable base course.
The basic material requirements for aggregate are listed in Table 1 for crushed aggregate base
course requirements.

The gradation of the crushed aggregate base course must meet the grading requirements
given when tested per ASTM C117 and ASTM C136. The gradation should be properly graded
from coarse to smooth and not vary from a lower limit to one boundary to an adjacent boundary
or vice versa. Crushed Aggregate grading requirements are as listed in Table 2.

The fraction of material that passes through the No. 200 sieve should not be more than
2/3 of the fraction that passes through the No. 40 sieve.

5. Stage of Work
5.1 Mixing Formula
Do the mix together with the consultant as follows:
1. According to the result of DMF from approved independent test with the results as
below:
a. Fraction 0 – 19 mm = 55%
b. Fraction 19 – 37 mm = 45%
2. Mixing done with equipment:
a. Loader as the mixing measurement with bucket capacity of 3 m3
b. Excavator as the mixer
3. Mixing done with equipment:
a. 0 – 19 mm = 11 bucket
b. 19 – 37 mm = 9 bucket
c. Mix the fraction 0 – 19 mm as much as 11 bucket and fraction 19 – 37 mm as
much as 9 bucket with excavator.
4. After the mixing is done, then take the sample to continue in sieve analysis two time a
day. If the sieve analysis got into gradation envelope, continue hauling to base course
location.
5. Every 5000 cu.m base course aggregate production needs to be checking for aggregate
properties requirements.
5.2 Preparation Work
Detail checklist form can be seen below. Breakdown of the checklist form will be more
completed following condition at site construction on Attachment section. Documents
submitted before starting work are as follows:
1) Approved Material test document, design mix formula (DMF)
2) Approved Work method statement (WMS)
3) Approved Shop drawing
4) Job safety analysis (JSA)
5) Request for Inspection (RFI)

6) Request for Work (RFW)


- Weather conditions in the field and in the quarry are not raining heavily (rain intensity
is not more than 40 mm/hour).
- Ensure that the field conditions are not wet.
5.3 Working stage as follow
The following of the working method can be seen in the flowchart below:

a. Ensure that degradation in preloading area has stopped on runway and shoulders. Sequence
of removal preload start from SP1, SP2, SP8, SP9, SP10, SP11, SP12, SP14, and SP15.

b. Stacking out MC0 survey on top preloading level prior to preloading work STA 112+360 -
112+810 as per airside requirement.
c. Excavation for thickness levels > 7.5 cm using an excavator. Excavators is used to level the
top basecourse to achieve elevation according to shop drawings. Operators will be briefed to
avoid excessive cuts and unnecessary distractions.

d. The results of the preload material remove are converted from bucket to rites (6 buckets for
1 rites) with capacity bucket 2-2.4 m3, where 2m3 for flat condition and 2,4 m3 for high
condition of bucket. In the next calculation, the bucket capacity is used 2m3. The determine
material used for the all area access road air side (Wika and MGM Phase 2).

6. Schedule
The work of Removal Preloading on Zona D runway and shoulder requires a work
duration of 63 days.

7. HSE Concern
1. Make sure that all employee good health and complete with rapid antigen test in Kediri.
2. Conduct HSE Induction for all new employee and provide ID cards before trial
compaction.p
3. Use the mandatory PPE, there are helmets, safety glasses, masks, vests, and safety
shoes.
4. Ensure all heavy equipment to be used has been inspected by HSE and Mechanical
Personnel.
5. Ensure SILO and SIO of heavy equipment and driving license (SIM B2) for dump truck
drivers are appropriate and still valid (not expired).
6. Create Job Safety Analysis (JSA) of Subgrade Preparation work, and socialization to
all employee.
7. Installing rain gauge to record the intensity of rainfall over a certain period of time.
8. Installing tents used for workers to rest in the field and take shelter from the sun and
rain.

9. For night construction using lighting lamps to illuminate the site construction
10. Rotary lamp used for all vehicles and heavy equipment works at night.
11. Conducting HSE Morning Talk and toolbox meeting before starting work, and all
employee must attend.
12. Doing good housekeeping in the work area.
13. There supervisors and safety officer standby in the field.
14. Making a request for work subgrade preparation and approved by PMCM.
15. Installed warning signs for Danger and “Restricted Areas”.
16. Installed the barricade around the Base Course work site.
17. There are signs for evacuate directions and provide the first aid bag equipment at the
work site.
18. After completing the activity, a barricade line will be installed and sign boards are
prohibited from crossing the “restricted area”.
19. If medical attention needed, will be treated in clinic dr. Vitis Jl. Jawa No.262 B RT/RW
003/002 Dusun Sembak, Grogol, Kec. Grogol, Kabupaten Kediri, Jawa Timur 64151
(attachment flow emergency evacuation).
