JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, HERIBERTO TEPACIA PANUGAN and REYNALDO BARING

CABARCE, Filipino, of legal age, with address at Sitio Alaska Viking,
Mambaling, Cebu, after having been sworn in accordance with law, state that:

1. We have known MARK ANTHONY JAGAPE GO for more than

eleven (11) years now;

3. In his Social Security System record, his name was inadvertently

entered as MARK ANTHONY UY GO, which is clearly an erroneous entry;

5. Her correct name in his record of birth is MARK ANTHONY

JAGAPE GO;

6. We hereby attest that the person who appears with the name MARK
ANTHONY UY GO, and MARK ANTHONY JAGAPE GO is one and the same
person;

7. We execute the foregoing affidavit in order to attest to the

truthfulness and the veracity hereof, and for all other legal intents and purposes;
