Multidisciplinary team interventions with the participation of the hospital or primary care pharmacist.

Participants/population
The study population will be chronic adult patients cared for by a multidisciplinary team with a pharmacist
involved.

Control group will included the standard of care

Main outcome(s)
The most important outcome is hospital admission. This outcome is defined by admission of a patient to
hospital or assistance in an emergency department.

Data extraction (selection and coding)


We will select the studies by titles first and then by abstracts.

Three reviews authors will identify potential studies.

If there is any doubt, external reviewer will decide if the study is to be included or not.

Two review authors will extract data independently.

Risk of bias (quality) assessment


Two review authors will independently assess the risk of bias in included studies by the analysis of the
quality of the studies with the NIH tool - Quality Assessment of Controlled Intervention Studies.

Strategy for data synthesis


We will provide a narrative synthesis of the findings from the included studies, structured around the type of
intervention, target population characteristics, type of outcome and intervention content. We will provide
summaries of intervention effects for each study by calculating odds ratio for drop outs risk of hospital
admission and or standardised mean differences for Quality of Life. For all outcomes, trial results will also be
combined using a random effects model to test robustness to model choice.
