APPRENTICESHIP AGREEMENT

This AGREEMENT duly entered into this _____________________ day of _______, 2008 by and between
Movie Star (Phils.) Inc. with business address at Ortigas Avenue Extension KM 26 Bo. Dolores Fibertex
Compound Taytay, Rizal hereinafter referred to as the FIRST PARTY,
and_____________________________________________ born on
__________________________________ and residing at
________________________________________________________________________hereinafter
referred to as the SECOND PARTY, and being minor is represented by his ____________________ hereinafter
referred to as the GUARDIAN

WITNESSETH: That the FIRST PARTY and the SECOND PARTY, represented by his GUARDIAN, do
hereby mutually agree and bind themselves as follows:

1. That the FIRST PARTY agrees to develop and train the SECOND PARTY and the second party agrees to
apply himself diligently and faithfully as an apprentice subject to the following terms and conditions:

2. That the schedule of apprenticeship shall be in accordance to the course design hereto attached and made
part of this Agreement and marked as Annex ”A”

3. That the SECOND PARTY shall work the equivalent of eight (8) hours a day in the course of his
apprenticeship. Overtime work may be allowed provided that the hours of work rendered are duly credited
towards hours of training with the effect of shortening the period of apprenticeship. The SECOND PARTY
shall be entitled to the corresponding overtime work pay.

4. That during the probationary period, either party to this Agreement may terminate said Agreement by
serving written notice on the other at least (5) working days before the actual date of termination, stating the
reason thereof.

5. That if the SECOND PARTY fails to perform satisfactorily within any stage of his training, he may be given,
at the discretion of the apprenticeship committee, a period of one (1) month within which to improve his
performance. If within this period the SECOND PARTY fails to show any improvement, he shall be dropped
from the program by the FIRST PARTY.

6. That after the probationary period, either party to this Agreement may terminate the same only for any of the
valid causes mentioned in Section 25, Rule VI, Book II of the Rules and Regulations Implementing the
Labor Code.

7. That upon satisfactory completion of the Apprenticeship Program by the SECOND PARTY, the
Apprenticeship Committee shall recommend to TESDA Provincial Office concerned the issuance of
Certificate of Completion. Graduates of Apprenticeship Program shall be subjected to competency
assessment. Upon passing, a corresponding Certificate of Competency shall be issued by the FIRST
PARTY to the SECOND PARTY.

8. That the company rules and regulations shall be observed as part of the terms and conditions of this
Apprenticeship Agreement.

9. That this Agreement shall take effect on the date of the start of training by the Apprentice.
