CONTRACT OF SERVICES

KNOW ALL MEN THESE PRESENTS:

This contract is made and entered into this 4 th day of JANUARY, 2020 and between LADE
ENTERPRISE., a business enterprise solely- owned by ELIZABETH LADAO DEOLDO of legal age,
Filipino and resident of Poblacion, Tampakan, South Cotabato hereinafter referred to as the FIRST
PARTY AND LIBERTY ELEMENTARY SCHOOL, Liberty, Tampakan South Cotabato a duly
established government agency with Office address at Liberty, Tampakan, South Cotabato
hereinafter referred to as the SECOND PARTY.

WITNESSETH
That the FIRST PARTY and the SECOND PARTY in consideration of the mutual covenants
and agreement made herein, by one to the other, by these presents, entered into contract of service
hereby by FIRST PARTY Render Shared Internet services to the SECOND PARTY on the following
terms and conditions, to wit:

1. The FIRST PARTY shall perform the services of the SECOND PARTY in BY
PROVIDING shared internet service to the Danlag National High School for the period
of twelve ( 12 ) months commencing October 2019 to September 2020 with a monthly
bill of 1,180.00 chargeable against School MOOE;
2. This contract shall cease to be kin any force and effect in the event that the SECOND
PARTY shows failure to pay the monthly bill.
IN WITNESS WHEREOF, parties hereto have hereunto signed this contract on this 1st day
of October, 2019 at Danlag National High School, Danlag, Tampakan, South Cotabato.
