CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:

This contract made and entered by and between:

ANIBONGAN NATIONAL HIGH SCHOOL, a public school and under jurisdiction of Department of
Education, Surigao del Sur with official address at Anibongan, Lianga, Surigao del Sur, represented herein
by the School Head JEAN G. COQUILLA, herein after referred to as the “First Party”
-and-

ANDRES OZON , of legal age, Filipino and with residence/office address at Poblacion , Lianga, Surigao del
Sur, herein after referred to as “Second Party”.

-WITNESSETH-

1. That the First Party is in need of the service of the Second Party who shall performed work not
performed by the regular personnel of the school.

2. That the Second Party has signified his/her intention to which the First Party has accepted to provide
the service needed by the later.

3. That Second Party hereby possesses the qualification, experience and skills required to perform the job
as described herein;

4. That the Second Party hereby attest that he is not related within the third degree of consanguinity or
affinity to the representative of the first party; that he has not been previously dismissed from
government service by reason of an administrative offense.

5. That in the view hereof, the Second Party is hereby contracted as a UTILITY of Anibongan National High
School for a period of (3 months ) three months starting from April 2019-June 2019 with a monthly
honorarium of Php. 4,500.00

6. That the UTILITY of the First Party is expected to perform the following functions:
(a.) Secure the School Premises;
(b.) Observe Clean and Green program of the school;
(c.) Observe and make necessary report to the school head and the proper authorities;
(d.) Perform special function as may be delegated by the school authorities.
7. That the Second Party shall perform work at a time and schedule to be agreed upon by both parties;
8. That it is understood that this contract does not create an employer and employee relationship
between the First party and Second party, that the services rendered hereunder are not considered and
as bases

8. Notwithstanding the duration of this contract, the service of the Second Party may be terminated earlier
than the expiration date due to unsatisfactory services, unavailability of funds, or other reasons within
the control of the Second Party.

IN WITNESS WHEREOF, both parties have hereunto set their hands this 29th day of
March, 2019 at Anibongan, Lianga, Surigao del Sur.
