CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This contract made and entered by and between:

The Municipal Government of Tubod, Lanao del Norte, herein represented by its
Municipal Mayor, HON. LEONCIO C. BAGOL, with postal address at Tubod, Lanao del
Norte, Philippines, hereinafter referred to as the “FIRST PARTY”;

ANJILLY A. IBRAHIM, of legal age, married, Filipino and a resident of Pigcarangan,

Tubod, Lanao del Norte, hereinafter referred to as the “SECOND PARTY”;
-and-
Pigcarangan Integrated School with school address at Pigcarangan, Tubod, Lanao del
Norte, herein represented by its School Head, HELEN C. TORRES, hereinafter referred to as
the “THIRD PARTY”
-WITNESSETH-
WHEREAS, the Second Party has signified his/her intention, to which the First Party has
accepted, to provide the service needed by the Third Party;

WHEREAS, the Second Party hereby possesses the education, experience and skills
required to perform the job as described herein;

WHEREAS, the Third Party consented that the services of the Second Party shall be
performed at Pigcarangan Integrated School, Tubod;

NOW THEREFORE, for and in consideration of the foregoing premises and of the
mutual covenants and stipulations herein set forth, parties have agreed as follows:

1. The Second Party is hereby contracted as TEACHER by the First Party for the month of
June 1, 2020 to June 30, 2021 in consideration of a monthly rate of Php __________ to
be paid at the end of each month by the latter;
2. The Second Party under the supervision of the Third Party is expected to perform his/her
function as Secondary Teacher in accordance with the DepEd Order, rules and
regulations;
3. It is understood that this contract does not create an employer-employee relationship
between the First Party, Second Party and Third Party;
4. The service rendered hereunder are not considered and will not be accredited as
government service, nonetheless, it may be credited as relevant teaching experience; and
that the Second Party is not entitled to benefits enjoyed by the regular personnel of the
First Party;
5. The Third Party gives its full consent that the services of the Second Party shall be
performed at Pigcarangan Integrated School; and
6. The Second Party shall perform work at a time and schedule to be agreed upon by
him/her and the Third Party;
