CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This Contract is made, executed and entered into, in this Municipality of Calauan, Province of
Laguna, Philippines, by and between:

The LUZONIAN CENTER OF EXCELLENCE FOR SCIENCE AND TECHNOLOGY (LCEST) INC. , Calauan,
Laguna, Philippines, an educational institution duly organized under the existing laws of the Philippines,
represented in this document by its School Authorized Representative, Rebecca M. Tripulca, Filipino citizen,
of legal age, married, and with residence and postal address at Lamot I, Calauan, Laguna, Philippines, and
may hereinafter refer to as the PARTY OF THE FIRST PART;

-And-

SANCHEZ CLINIC, represented in this document by its CLINIC Head, IMELDA S. MARFORI, likewise
Filipino with residence and postal address at 144 A. Marfori Street, Brgy. Silangan, Calauan, Laguna,
Philippines, and may hereinafter refer to as the PARTY OF THE SECOND PART…

WITNESSETH:

1. That, the Party of the First Part is an educational institution, with its principal office at 3 rd Flr. Bose,
Bldg, Brgy. Kanluran, Calauan, Laguna, Philippines, duly organized in accordance with the existing laws of
the country; Whereas, the Party of the Second Part is a CLINIC, represented by Physician who is actively
exercising her profession as such’;

2. That, the Party of the First Part manifested its willingness and desire to TIE-UP the services of the
Party of the Second part to accommodate the students of the said institution of learning during time of needs
or whenever deemed necessary; whereas, the latter signified his willingness to accept the former’s offer, and
the parties hereto bind themselves to the following term and conditions, to wit:

a. That, the Party of the First Part shall send their students to the Second Party whenever
deemed necessary and that the students will be liable for whatever expenses they will
incur;

b. That, the Party of the Second Part shall render accommodate any students enrolled
during the semester, eight (8) hours, a day, without necessarily maintaining an office
and/or clinic within the school premises;

c. That, the Party of First Part shall have a full right and privilege to terminate this
contract and terminate the services of the Party of the Second Part should it appear that
the latter is incapable of performing his duties and obligations as such, and

d. That, the term of this Contract shall be two (2) school years, which will begin on the
_1st day of July, 2022 and will end on the 30th day of July, 2024;

3.That, all of the provisions of this Contract are read, understood and confirmed by both the Party of
the First Part and Party of the Second Part, and they affirmed that all of the foregoing provisions will redound
to the benefit of both parties hereof.

IN WITNESS WHEREOF, the Party of the First Part and the Party of the Second Part have hereunto
affixed their signatures, this 22 nd day of July, 2022, in this Municipality of Calauan, Province of Laguna,
Philippines.
