AFFIDAVIT OF DISCREPANCY

(ONE AND THE SAME PERSON)

I, HANNA JOY TRINIDAD TAGOCTOC, of legal age, single, Filipino, and residing
at Camachile, Doña Remedios Trinidad, Bulacan after having been duly sworn to in accordance
with law depose and say:

1. That my real full name is as stated above HANNA JOY TRINIDAD TAGOCTOC who
was born on April 26, 1999, at Angono, Rizal;

2. That since childhood, I have been using and indicating my name as “HANNA JOY
TRINIDAD TAGOCTOC” and used the said name when I signed documents in school
and secured government identification card as could be seen in my National ID duly issued
by the Philippine Identification System (PHILSYs) hereto attached as Annex “A”.

3. That in my Certificate of Baptism it was also named “HANNA JOY TRINIDAD

TAGOCTOC”; copy of which is hereto attached as Annex “B”.

4. That I am coordinating with the Professional Regulation Commission (PRC) for the
processing of my application for Licensure Examination for Teachers (Bachelor of
Elementary Education), however I am required to present my Certificate of Live Birth
(PSA) as one of the requirements, but the name appeared in my Certificate of Live Birth
is “HANNA JOY TRINIDAD TAGUGTOC” which is hereto attached as Annex “C”.

5. That I am executing this affidavit to attest to the fact of the foregoing; that my real full
name is “HANNA JOY TRINIDAD TAGOCTOC”; that the full name “HANNA JOY
TRINIDAD TAGOCTOC” and the name “HANNA JOY TRINIDAD TAGUGTOC”,
refers to one and the same person, both referring to me to support my report with
Professional Regulation Commission (PRC) and for all intent and legal purposes it may
serve.
