Sub: Quotation cum Term Sheet of …………… module.

Dear Mr. ……….

I Sanjay Sethia providing a ……………. Module of Life Time service for Tally
against single serial No. for the charges of Rs. ………../- (Rupees …………….)
only.

TERMS & CONDITIONS:

1. That if the company is same with different address in single Tally Serial
No. the service will be for Life Time (till the expiry of the License).
2. That if the company is same and address with same module the charges
will be Rs. 4,000/- (Rupees Four Thousand) only with the life time
service.
3. That if the company is different with different Tally Serial No. then the
TCP cost will be Rs.10/-.
4. That the AMC as per the terms and conditions with developer Rs.7,500/-
but on request this has granted as a special one with the cost of Rs.
5,000/- though its requested that the first and second year AMC be
charge as Rs. 2,500/- per year and from third year the charges will be
Rs.7,500/- but due to disagreement and agreed that the charges has
been fixed with Rs. 5,000/- per annum for the First two years and from
third year Rs. 7,500/- will be chargeable. (GST Extra as applicable).

Services that will be provided on the AMC:

i. Any type of default service will be solved in between 10 am to 6 pm
ii. For any query and emergency service call in between 9 to 10 pm.
 5. That the person/company liable to pay the TCP fees as per the rates of
Rs. …………. per annum for taking the service with the condition that the
fees/rates can be reviewed per annum on mutual understanding.

PRIVACY:
That it is understood that all reasonable steps will be taken to protect the
privacy and / or confidentiality in accordance with applicable privacy laws
when discussing with or receiving information from my carer or representative
in relation to my care and service needs.

That it is acknowledged that you have read and/or had explained before you
the contents of this TCP Module information and agreement booklet and agree
to receive

That it is understood that the written care plan and discharge plan form part of
this term sheet and may be amended over time as mutually agreed.

That it is understood that this agreement can be reviewed at any time and
changed with mutual consent. It will be notified in writing of any change by
either party (if necessary).

TERMINATION:

That it is understood that either party can terminate this agreement by

giving .......... day notice.
