• Balloon angioplasty and stenting exercise complex

- Carotid stenting
It is required to perform carotid stenting during the exercise. This kind of intervention is of
endovascular-type procedures and is aimed at placement of a special tube (stent) into blocked
carotid artery by atherosclerosis, that is a systematic process of atherosclerotic plaques occurrence
on the arterial inner wall. These plaques consist of cholesterol, calcium and fibrous tissue.
For a given time it is required to perform the main stages of the intervention
that include preliminary diagnostic angiography, dilatation of a blocked part,
placement and opening of a stent or a balloon, control angiography, extraction of the instruments.
It is important not to break vessels’ integrity and not to exceed the quantity of contrast being injected.

- Renal stenting
It is required to perform renal stenting during the exercise. This method of endovascular
angioplasty includes renal artery opening by estuarine and non-estuarine stenosis,
renovascular hypertension and fibromuscular dysplasia by insertion of a plastic balloon catheter
and a special tube (stent). For a given time it is required to perform the main stages of the intervention
that include preliminary diagnostic angiography, dilatation of a blocked part, placement and opening
of a stent or a balloon, control angiography, extraction of the instruments.
It is important not to break vessels’ integrity and not to exceed the quantity of contrast being injected.

- Coronary intervention
It is required to perform coronary intervention during the exercise. This procedure belongs to the
Coronary heart disease treatment and opens blocked artery by placing and opening of a special tube
(stent) in the narrowed part. For a given time it is required to perform the main stages of the intervention
that include preliminary diagnostic angiography, dilatation of a blocked part, placement and opening
of a stent or a balloon, control angiography, extraction of the instruments.
It is important not to break vessels’ integrity and not to exceed the quantity of contrast being injected.

- Iliac stenting

It is required to perform iliac stenting during the exercise. The process of endovascular balloon
stented iliac arterial opening with placement of a special tube (stent) lets restore optimal blood supply
of femoral and iliac zones and blood supply to the lower extremities. For a given time it is required
to perform the main stages of the intervention that include preliminary diagnostic angiography,
dilatation of a blocked part, placement and opening of a stent or a balloon, control angiography,
extraction of the instruments. It is important not to break vessels’ integrity and not to exceed the quantity
of contrast being injected.

• С-arm movement and positioning
- Coronography projections
The knowledge of standard angiographic projections of coronal arteries is being tested:
-Right anterior oblique projection with a caudal angulation;
-Right anterior oblique projection with a cranial angulation;
-Left anterior oblique projection with a caudal angulation;
-Left anterior oblique projection with a cranial angulation;
-Left oblique projection without angulation;
-Right oblique projection without coagulation;
-Left oblique projection with a cranial angulation.
Each projection gives an opportunity to examine a separate arterial segment. The task is to get
Information on the state, anatomy, morphological structure and changes presence in coronal
arteries along with the accurate indication of affections’ localization and developmental character.

- Prevalent angiographic projections


The knowledge of four standard angiographic projections for a selected affected area is being tested:
-Frontal anterior projection;
-Back anterior projection;
-Right lateral projection;
-Left lateral projection.
The task is to get information on the state, anatomy, morphological structure
and changes presence in coronal arteries along with the accurate indication of affections’ localization
and developmental character.

- Control of a camera with different viewing angle (0°, 30°, 45°);


The task is to find several objects and capture them for a defined period. Angled laparoscopes (30 and 45 degrees)
are being used.

- Improvement of instruments coordination in space


For development of coordinated movements of right and left hands. The task is to catch the ball with
a selected instrument by right and left hands in turn and to place it into the box holding the box top by
the second hand. The balls are located throughout the whole area and are moving permanently.

- Vessels clipping and capturing


Artery clipping could be complicated because of the elasticity and fragility of the vessel. Duct located in the front
is limiting the access to it as well. Size of a zone being clipped and time for clipping could be changed by
an administrating instructor

- Endoscopic scissors handling


Endo-scissors handling skills training – incision of the tissue in the required direction within
the specified limits. Tissue is being clutched and strained in the required direction with a grasper and
is being incised with scissors.

- Endoclip applicator handling


The task is aimed at the basic skills of endoclip applicator handling in non-anatomical environment.
There are parallel vessels with blue-marked places for clips to be placed onto in the training room. For execution
of the exercise it is required to clip all the vessel in any order for a given time trying to apply clips correctly
herewith.

- Electrocoagulation operating skills training


The task is designed for development of initial skills of safe coagulation with a help of the endo-hook or dissector.
It is required to clutch a spherical object, pull it off and uncover a yellow bunche (commissures) connecting it to
the stake. Every bunch is to be intersected with the instrument in a sequential order. It is forbidden
to damage the sphere and main threads keeping the sphere.

- Moving and capturing of objects in space


The task is designed for development of coordinated hand movements and instrument rotation skills.
An object is to be caught, turned and placed according to the tip (contour color shadow within the room).
Objects are to be superimposed. 6 different objects are available.

- The task on pins’ movement

Pins are to be picked up and placed into holes.


A high complexity level and accordingly level of learned skills could be achieved due to the holes’ orientation,
their size regarding the pins, and pins’ fragility.

- The task on movement of the objects on pins

Pins with different color of the bases are located in the box. The task is to put every ring on the pin, the color
of both pin and ring should coincide. The rings could be located at every part of the room.

• Certain significant laparoscopic skills:


- Endoclip applicator handling
It is designed to develop main skills in handling of endoclip applicator in non-anatomical environment.
Vessel simulations are located at different heights in the training room. From time to time
small area that must be clipped is turning into blue. Time for the application of each clip
is limited. Purpose of the exercise is to clip as many vessels as possible, trying to apply clips correctly herewith.

- The task on possession of endoscopic scissors in training and exam mode


The task is designed for development of basic skills of handling of the endoscissors and both hands
coordination improvement. A tissue with marked closed contour is located in the centre of the training room.
The purpose of the exercise is to cut the tissue along the contour. The incision should be made so as
the tissue between branches is exactly inside the marked contour. For more precision work another instrument
should be used to hold the tissue. The exercise finishes automatically when incision is completed.
The following tips are available in the training mode: highlighting at incision out of the contour,
arrows on the instruments in case they disappear from the camera view, time-left messages. There are
no tips in the exam mode

• The complex of training tasks on suturing and knotting

- Double knot tying for the right hand


The exercise is designed to develop skills of intracorporeal tying of a simple parallel knot with
two loops with the right hand. The needle with the thread already passed through the tissue is located
in the room. It is required to tie the knot with the right hand. Except for the total time limit of the exercise,
definite time is allotted for each knot, on the expiry of that time the knot is considered to be untied.
Scores are given for correctly tied knots; score is lowered for tying a knot different from task-required or
in case if the passage direction of loops was not changed.

- Double knot tying for the left hand


The exercise is designed to develop skills of intracorporeal tying of a simple parallel knot with
two loops with the left hand. The needle with the thread already passed through the tissue is located
in the room. It is required to tie the knot with the left hand. Except for the total time limit of the exercise,
definite time is allotted for each knot, on the expiry of that time the knot is considered to be untied.
Scores are given for correctly tied knots; score is lowered for tying a knot different from task-required or

- Z-stitch overlaying
The exercise is designed to develop skills of suturing in non-anatomical environment.
Make the required number of Z-sutures following the tips (highlighting of grasping sites and the applied instrument,
tracing of correct motion trajectory of the instrument). Each suture is limited in time, on the expiry
of the given period current suturing is considered to be a failure, and all the objects are being brought to the initial state.
Scores are given for number of correct sutures.

- Mattress suture overlaying


The exercise is designed to develop skills of suturing in non-anatomical environment.
The required number of mattress sutures should be made following the tips (highlighting of grasping sites
and the applied instrument, tracing of correct motion trajectory of the instrument). Each suture is limited in time,
on the expiry of the given period current suturing is considered to be a failure, and all the objects are being
brought to the initial state. Scores are given for number of correct sutures.

- Drill orientation of a needle in the needle holder


The task is designed to develop basic skills of needle manipulation.
It is required to grasp the needle by the highlighted instrument. Then use the second instrument
to grasp the thread, and manipulate the thread to place the needle into the set position
(semitransparent contour shadow of needle). This procedure must be performed the required number of times
within the exercise, each attempt is limited in time. Scores are given for the number
of correctly positioned needles.

- Needle-suturing with the right hand


The task is designed to develop skills of tissue suturing with the right hand.
A soft object with several prepared zones (circles) for sequential suturing is located in the room.
It is required to follow the displayed messages and to insert the needle into the circle at the inlet
and to take it out through the opposite circle. Suturing is performed the set number of times at the left and then
is repeated at the right. Scores are given for percent of correct sutures.

The exercise is designed to develop skills of suturing in non-anatomical environment. The required number

given period current suturing is considered to be a failure, and all the objects are being brought to the initial state.

-Z-suturing
The exercise is designed to develop skills of Z-suturing. The required number of Z-sutures should be made

• Practical skills of laparoscopic cholecystectomy

- Performing a full course of the operation of laparoscopic cholecystectomy


The task is designed to improve skills of laparoscopic cholecystectomy use in anatomical environment.
Main stages should be performed in the process of surgical intervention:
- Traction (access provision to the operated zone)
- Abdominal membrane incision;
- Clip application onto the cystic artery and the cystic duct, and their subsequent cut;
- Removal of the gallbladder from its bed and its mobilization;
- Aspiration-irrigation.
In case of any complications, it is required to eliminate them promptly
(clip the damaged vessel, coagulate bleeding, etc.)

- Traction and abdominal membrane insicion


During the task one of the stages of laparoscopic cholecystectomy is being exercised
in anatomical environment. It is required to perform traction for convenient bladder neck mobilization.
Commissures (if any) and abdominal membrane are to be incised along
the gallbladder bed’s anterior and posterior surfaces throughout one third of the body and neck.
In case of any complications, it is required to eliminate them promptly
(clip the damaged vessel, coagulate bleeding, etc.)

• Practical skills of laparoscopic cholecystectomy

- Dissection of the structures in Calot’s triangle


During the task one of stages of laparoscopic cholecystectomy is being exercised.
During the task execution it is required to incise abdominal membrane gradually starting
from the bladder neck to the choledochous duct, grasping and raising it with electrosurgical
hook or dissector, exposing thereby the Calot’s triangle area. In case of any complications,
it is required to eliminate them promptly (clip the damaged vessel, coagulate bleeding, etc.)

- Clipping and the intersection of the cystic duct and artery


During the task one of stages of laparoscopic cholecystectomy is being exercised.
During task execution it is required to apply 2-3 clips on the cystic artery first, and then to cut it
near the gallbladder wall (between the clipped sections). Apply clips onto the cystic duct and
cut it by scissors without coagulation. it is required to perform the task in accordance with the operation sequence
noted above to complete the exercise. In case of any complications, it is required to eliminate them promptly
(clip the damaged vessel, coagulate bleeding, etc.).

• Practical skills of laparoscopic cholecystectomy

- Gallbladder mobilization

During the task one of stages of laparoscopic cholecystectomy is being exercised.


It is required to separate the gallbladder from its bed and to remove it out of the abdominal cavity.
Hold the gallbladder and separate it from the liver using electrical tool. Then, after full separation,
use endobag-evacuator to remove the gallbladder. Coagulate bleeding on the liver, perform
aspiration-irrigation. It is required to perform the task carefully, trying to avoid bad errors
(excessive coagulation, etc). In case of any complications, it is required to eliminate
them promptly (clip the damaged vessel, coagulate bleeding, etc.)

• Laparoscopic appendectomy

- Complete laparoscopic appendectomy


It is required to perform laparoscopic appendectomy in anatomical environment.
It is required to perform all stages sequentially and carefully, trying to avoid bad errors
to complete the exercise.
If any complications occur, they should be eliminated.

• Total hysterectomy

- Total laparoscopic hysterectomy


The exercise is intended to enhance skills of total hysterectomy.
It is required in anatomical environment to remove the uterus together with uterine neck.
The surgery includes the following main stages:
- Cutting of ligaments and dissection of the urinary bladder;
- Uterine vessels hemostasis;
- Dissecting the uterus from vaginal walls away;
- Extraction of gross specimen;
- Tests of hemostasis.
It is required to perform all stages sequentially and carefully, trying to avoid bad errors
(surgery stages irregularity, excessive coagulation, etc.) to complete the exercise.
If any complications occur, they should be eliminated.

• Skills for gynecological surgery

- Tubal sterilization
The exercise is designed to enhance skills of laparoscopic sterilization.
It is required to block fallopian tubes by applying clips with Filshie clip-applicator.
Use uterine manipulator to move uterus aside, apply clip to the tube, then move uterus to the other side,
and clip the second tube. Make all stages of the surgery carefully step by step,
avoid damaging organs and tissues. In case of complications, it is required to eliminate them promptly.

- Ectopic pregnancy
The exercise is designed to develop skills of laparosopic surgery at ectopic pregnancy.
It is required to perform all stages (fallopian tube incision, ovum extraction, removal of the ovum out of
the abdominal cavity) sequentially and carefully, trying to avoid bad errors
(surgery stages irregularity, excessive coagulation, etc.)
In case of complications, it is required to eliminate them promptly.

- Prophylactic oophorectomy
The exercise is intended to develop skills of oophorectomy.
It is required to separate the ovary from the tube, to remove it from the abdominal cavity
using endobag-evacuator, and to perform tests of hemostasis. In case of complications,
it is required to eliminate them promptly.

• Diagnostic colonoscopy

The module is designed for training of practical skills of colon examination. During the training
trainee will learn how to handle the endoscope in a correct way, will learn the methods of a full detailed
examination, identification of different complicated anatomical cases and pathologies,
will try use of instruments for sampling of a tissue. Special attention is given to the technique of painless
colonoscopy and early cancer detection.

• Therapeutic colonoscopy
The module is designed for training of different practical skills in colon treatment. On full examination
trainee has to make polyp ablation, hemostasis, can find out diverticula or obstruction along with different
kind of pathologies. Having completed all the exercises trainee will learn how to identify the pathologies
in a correct way, make decision on treatment methods, arrest the occurring complications, use
different drugs and prescribe further treatment.

• Sigmoidoscopy

Exercises of that module are designed for examination of the rectum and sigmoid colon.
