Activity: Nitrogen Helium Leak Test on the Test Package 16, Train A & Train B 1 LP Compressor Discharge and

Intercooler.

Description: (Describe the event)

On the above date and time, our work group was tasked to perform N2HE Leak Test on Test Package 16, Train A & Train B

1 LP Compressor. Upon performing the necessary checks and obtaining the relevant permit to work, we started

to pressure up the system. During pressurization to 25% (6.4 bar), Pressure Relief Valve on Train A & B were lifted.

Upon hearing the popping sound, night shift Supervisor stop the pressurization immediately and check with the Pump

Operator at the Quay Side. However, no pressure was shown on the pressure gauge located at the monitoring point.

Supervisor performed additional checks on the system gauge and discovered there were 28 bar in the system. Due to

testing was ceased to identify the cause.

Causes leading to the event:

1. The hose connection is connected to diaphragm seal not directly to the double block bleed valve. Since it is

connected to the diaphragm seal, it can only sense the pressure thus not allowing the process air to flow through for

remote sensing purpose. Due to that, no reading was detected remotely.

What immediate action was performed to make location/area safe:

1. Depressurized the system to 0 bar.

2. Inform relevant parties on the occurrence of the event.

3. Conduct tri party meeting to discuss about the event.

Actions to be taken to ensure safe operation of activity:

1. Review of all test packs to determine the location of tie - in points (injection / return).

2. Installation of additional gauges in two more location (one at OPP, one at quayside (pump).

3. Generate and share lesson learnt for work group.
