REQUEST OF VERIFICATION TRANSCRIPT OF NURSING EDUCATION

I’m Dharshalinie A/P Elongkovan from cohort 19, would like to request college to send
directly my transcript of nursing education to Registrar, Singapore Nursing Board, 81 Kim
Keat Road, #08-00, Singapore 328836 or email directly to SNB@spb.gov.sg. Please make
sure the verification of education transcript is in English and to be printed out in the School
letter and to be send by courier mail in the school envelope. Kindly ensure that the transcript
reflects the detailed breakdown of all theory subjects in theory hours and clinical practise
hours.

2. Therefore, I would kindly wish to bring to your attention a humble request to issue the
documents. Without these verification documents, I fear that this job will be nothing but a
mere false start to an already condemned career. My future is, fortunately, in your very
capable and committed hands.
