JOINT – AFFIDAVIT OF
TWO DISINTERESTED PERSONS
(Application: Late Registration of Birth Certificate)

We, Virgilio S. Bacalso and Soliven Remedios Sapine, both of legal age,
Filipinos, both married and both residents of Yati, Liloan, Cebu, Cebu, after having been
duly sworn in accordance with law, do hereby depose and say, T H A T:

1. We personally know parents, Laurencio Brua and Patrecia Dela Peña and their
child Manolo Dela Peña (Manolo, for brevity) because we are friends and
neighbors for a long time.

2. Manolo was born on February 11, 1963 at Tabuelan, Cebu. His birth certificate was
not registered within the reglementary period for reason that the family moved
residence several times that the hectic schedule affected the registration process.

3. We are not in any way related to Manolo Dela Peña by affinity nor consanguinity;

4. That finally, we are executing this affidavit for the purpose of informing the
authorities especially the Local Civil Registrar and the Philippine Statistics Authority
of the above-mentioned and to support the application for late registration of birth
certificate or DELAYED REGISTRATION of Manolo Dela Peña who is the child of
Laurencio Brua and Patrecia Dela Peña. This affidavit is based on personal
knowledge.

IN WITNESS WHEREOF, we hereby affix our signatures, this April 27, 2022 at Liloan,
Cebu, Philippines.
