Subject Project Report – Successes

INTRODUCTION

This project report is aimed at demonstrating technical competence and understanding of construction health and safety planning,
implementation and management; as relevant to the Construction Health and Safety Agent level of professional registration being
applied for. Furthermore, this project report focusses upon the successes that I achieved on the project and how I achieved them.

This project report is made with reference to On Grand central ext 8 was contracted in stage one for the multi storey housing unit.

This project report is made in terms of the 9 (nine) knowledge areas, and, the relevant scope of services and deliverables within each
knowledge area.

1. PROCUREMENT MANAGEMENT

I assisted the client in developing the SHE section of the procurement process for selection of equipment, services and suitable
competent contractors who can deliver a safe and healthy contract. In planning I considered available information, including scope of
work, list of services needed, risk profile, baseline risk assessment and project health and safety specification and tender information
history.

The process included - documented internal procedures, minimum standard requirement, requirement for staff involved in tender,
internal training, tender rules, tender period, SHE bill of quantities, tender clarification meeting procedures, issuing of tender, receipt
and opening of tenders, document control system to ensure tender evaluation security, list of information(contractors history,
management systems, competence) that must be returned by contractor submitting a tender, tender result report and process review.

I successfully assisted in developing a documented procurement process, training requirement for evaluation team, implemented the
program, evaluation tenders received, evaluation report and recommendation to the client.

2. COST MANAGEMENT

On this project, after spending time in consultation with my client CEO and comparing his and my own experiences on similar
project that we have worked on.

I looked at the SHE costs bill of quantities of previous projects, the current project design and risk profile, scope of work and
historical information from contractors and suppliers. From this information I drafted a project specific SHE costs that will be
incurred on the new project.

The costing element that were considered included: SHE related surveys, geo-technical, environmental, traffic management,
training, PPE, SHE appointments, SHE agent services, cost of COID, SHE documentation including baseline risk assessment,
specifications and meetings, workers medical evaluation, emergency equipment, audits and inspections, safety signage,
barricading/edge protection, use of scaffolding, security services and site fencing.

With this information available, I advised my client to estimate the preliminary cost between 1 and 1.5% of total project cost. This
information was used in the initial SHE cost budget and final project SHE budget was 1.32%.

3. HAZARD IDENTIFICATION MANAGEMENT

I prepared a list of surveys that should be completed to identify possible hazards/ risks related to the construction project that could
have a negative effect on the health and safety during construction. I met with my client and discussed the advantage of identifying
these hasards/risks through surveys in the initiation fase of the project.
The list included:
Geo-technical which would establish ground conditions and ground structure
Environmental surveys will identify possible damage to vegetation, waterways, wetlands.
Traffic surveys will assist in traffic management planning needs.
Security surveys will assist in planning security needs.
Survey of existing services, i.e. electrical cables, sewage, storm water, gas, old dump site, Underground storage tanks, etc,
Contaminated land survey would prevent exposure to health hasard of workers and future home owners.
My client used this information I had provided and scheduled the appropriate surveys and discovered an old underground fuel tank.
The tank was successfully removed by a contractor who were equipped and experienced in this in this activity.

4. RISK MANAGEMENT

I developed SHE procurement process designed to identify and eliminate potential PC with a high risk profile which consists of a bill of
quantities that requires that the PC price for all SHE requirements related to their scope of work and a questionnaire focussing on 4
SHE aspects, i.e. HIRA, site management, Incident management and emergency preparedness and failure in one of these disqualifies
the contractor

The HIRA section of the questionnaire requires information and documented proof including: previous external audit results, 12 month
incident statistics, Issue based risk assessment, continuous risk assessment including daily activity specific risk assessment,
inspections, plant maintenance, workers medicals, SHE plan, fall protection and prevention plan, management appointments and
training.

I assisted in evaluating of the tender information based on price, completeness and adequacy of SHE including their risk profile and
compiled a report with recommendations which was forwarded to the procurement manager and client to assist in making an informed
decision when selecting the PC.

The implementation of these procedures resulted in the appointment of a PC who completed the project with no reportable incidents,
on time and within budget.

5. ACCIDENT OR INCIDENT MANAGEMENT

6. LEGISLATIONS AND REGULATIONS MANAGEMENT

I met with the designers after the commencement of phase 2 of the project to update them on the requirement of the construction
regulation 2014. I advised them on their legal responsibility and liability in design during construction related to safety in
constructability, maintainability and operability of the structure

I discussed in detail the Construction regulation 2014 requirement for designers


designing for safety including:

a) including ergonomic principals in their design


b) excluding materials and construction methods that could pose a hasard during construction and for maintenance after
completion of the structure.
c) Their responsibility in advising the client on required surveys which could prevent exposure to unforeseen hasards and risks.
d) the responsibility when mandated to inspect the structure during construction, stopping any work that is unsafe and not to
standard and at completion to ensure that it is safe for occupation.

This information was well accepted by the designers and used their execute their duties and during the construction fase I
monitored design risks management onsite in consultation with the designers.

7. HEALTH, HYGIENE AND ENVIRONMENT MANAGEMENT

The final SHE specifications which is an ideal opportunity to ensure that the PC who is making a bid for work has made adequate
provision for Health, hygiene and environment management on the project.

The final specifications which was included in the procurement document included: a) exposures to noise, dust, hasardous chemicals,
i.e. cement, paint, flammable liquids. b) surveys and assessment of ergonomic hasards. c) availability and adequacy of toilets,
drinking water, eating areas. d) management and control of waste, spills, emissions. e) monitoring, preventative and control
measures.
f) Job specific medicals.

On 20 June 2013 I was notified that DC electrical supervisor suffered an epileptic attack, sustained head injuries and passed away.

The investigation revealed the worker had no entry medical and did not reveal his status to his employer. At the date of the incident,
there were no direct requirement in legislation requiring medicals for all construction workers. The SHE specifications only required
medicals required by the Construction Regulation 2003.

After this incident I revised the baseline risk assessment and specifications to include medicals for all construction staff.

The changes were discussed with the PC and at the next project meeting and successfully implemented and monitored through site
audits.

8. COMMUNICATION MANAGEMENT

I setup a meeting with the principal agent, the client and designers to gather information of the expected work and related hasards and

risks during the defect liability period. After evaluation of information, I updated the baseline risk assessment and SHE specifications
and provided them with these updates.
After approval by the client and receiving the required input from the designers, I met with the PC SHE officer and discussed the
updated requirements and the PC responsibility in communicating the changes and manage the implementation by their
subcontractors. As there were various changes to the existing specifications and baseline risk assessment, we agreed to review their
file a week later to allow them to update their files.
I reviewed the file with the PC SHE officer and discussed the outstanding matters and finally approved the file.
I managed SHE during the defect period through monthly audits and weekly site inspections and discussing non-conformances with
the SHE officer and reporting the results to the client and at monthly meetings.

9. EMERGENCY PREPAREDNESS MANAGEMENT

The final SHE specifications which is included into the procurement documents is an ideal opportunity to ensure that the PC who is
making a bid for work has made adequate provision for emergency management on the project.

To finalise the SHE specifications, I used the current information available of the project including: scope of work, risk profile, draft
baseline risk assessment and draft SHE specification, results from surveys required in stage 1, feedback from designers,
geographical area of site and proposed duration.

The section of the specifications dealing with emergency preparedness management include: Legal requirement, emergency risk
evaluation, emergency plan and procedures, budgeting, staff training and competence, required equipment, emergency appointments,
emergency contact details, site evacuation plan, reporting, emergency alarms and drills.
