Executive Order No. 25 - MUNICIPAL DRINKING WATER QUALITY MONITORING COMMITTEE
WHEREAS, Code on Sanitation (PD 856) and its Implementing Rules and Regulations (IRR)
require drinking water to be protected from all types of contamination, subsequently, Philippine
National Standard for Drinking Water (PNSDW) was formulated based on this Code as a basis for
monitoring and surveillance of drinking water supply facilities.
WHEREAS, to carry out monitoring and surveillance activities, the IRR of Chapter II (Water
Supply) of PD 856 stipulated the creation of the Local Drinking Water Quality Monitoring
Committee (LDWQMC), in further support to the Local Government Code of 1990.

WHEREAS, Department of Interior and Local Government (DILG) and Department of Health
(DOH) has issued Joint Administrative Order 2022-002 which provided guidelines on
Establishing Local Drinking Water Quality Surveillance (LDWQS) Program through the Creation
of Local Drinking Water Quality Monitoring Committee (LDWQMC) as Mandated By the Code on
Sanitation (PD 856);
NOW, THEREFORE, I, SYMOND O. CAGUIAT, Municipal Mayor of Sta. Josefa, Agusan del
Sur, by virtue of the powers vested in me by law, do hereby -
Section 1. Creation. The Local Drinking Water Quality Monitoring Committee (LDWQMC) is
hereby created in the Municipality of Sta. Josefa, Agusan del Sur.
B. Specific Functions.
Municipal Mayor
1. Acts as chair of the Committee
2. Presides quarterly meetings or every committee meeting.
3. Signs the approved committee policies and resolutions.
4. Signs the approved annual work and financial plan
5. Delegates other related function.
Municipal Engineer
1. Informs the Committee on relevant civil works in the area that may affect the quality of water
sources.
2. Provides technical and logistic assistance to the committee
MENRO
1. Provides information to the Committee regarding incidence or occurrence of water.
contaminants arising from the domestic, industrial, commercial or institutional facilities.
2. Provides logistics and technical assistance to the committee.
NGO
1. Provides technical and logistics assistance to the committee.
2. Conducts information dissemination on drinking water quality
Section 4. Creation of TWG. The Technical Working Group shall be created also to assist the
LDWQMC and shall composed of the following:

Sanitation Inspector
1. Leads in the conduct of drinking water quality surveillance activities (i.e. sampling, sanitary
survey, etc).
2. Keeps report and records of the TWG and Committee.
3. Performs other functions as directed by the team leader.
Section 6. Meetings. The meeting of the LDWQMC shall be held quarterly or as often as
necessary.
Section 7. Secretariat. The Rural Sanitary Inspector shall act as the secretariat of the LDWQMC
and shall be responsible for providing technical support, documentation of proceedings,
preparation of reports and such other assistance as may be required.

Poblacion, Sta. Josefa, Agusan del Sur Philippines.
