AN EXECUTIVE ORDER ORGANIZING THE MUNICIPAL DRINKING WATER QUALITY


MONITORING COMMITTEE IN THE MUNICIPALITY OF STA. JOSEFA, AGUSAN DEL
SUR

WHEREAS, Code on Sanitation (PD 856) and its Implementing Rules and Regulations (IRR)
require drinking water to be protected from all types of contamination, subsequently, Philippine
National Standard for Drinking Water (PNSDW) was formulated based on this Code as a basis for
monitoring and surveillance of drinking water supply facilities.

WHEREAS, to carry out monitoring and surveillance activities, the IRR of Chapter II (Water
Supply) of PD 856 stipulated the creation of the Local Drinking Water Quality Monitoring
Committee (LDWQMC), in further support to the Local Government Code of 1990.
Government Code of 199;
WHEREAS, Department of Interior and Local Government (DILG) and Department of Health
(DOH) has issued Joint Administrative Order 2022-002 which provided guidelines on
Establishing Local Drinking Water Quality Surveillance (LDWQS) Program through the Creation
of Local Drinking Water Quality Monitoring Committee (LDWQMC) as Mandated By the Code on
Sanitation (PD 856);

NOW, THEREFORE, I, SYMOND O. CAGUIAT, Municipal Mayor of Sta. Josefa, Agusan del
Sur, by virtue of the powers vested in me by law, do hereby -

Section 1. Creation. The Local Drinking Water Quality Monitoring Committee (LDWQMC) is
hereby created in the Municipality of Sta. Josefa, Agusan del Sur.

Section 2. Composition. The LDWQMC shall be composed of the following:

Chairperson :HON.SYMOND O. CAGUIAT - Municipal Mayor


Vice Chairperson :MARY JOY J. TICAO – Municipal Health Officer

Members :ENGR. DIOSCORRO NAVARRO, JR – DOH Development Mgt. Officer


:MRS. OFELIA S. SIMUGAN – SJWDS Manager
:ENGR. JUVYLYN V. BITANG- Municipal Engineer
:HON. GINA LYN Q. PARBA- SB Com. Chair on Health & Sanitation
:DR. MIRALUZ M. CONVOCAR, DVM- Municipal Agriculturist
:MR. MABINSE M. PLAZA- MENRO- Designate
:ENGR. BIANCA NIOG- Provincial Sanitary Engineer

Section 3. Functions. The LDWQMC shall have the following functions:


A. Main Functions.
1. Endorses annual work and financial plan on water quality surveillance to the city/municipal
council.
2. Formulate local policies concerning implementation of water quality surveillance program.
3. Initiates regular or immediate sanitary survey for contaminated water supply sources.
4. Initiates the regular collection and analyses of water samples as prescribed within this
Administrative Order.
5. Reviews and approves the recommendation of TWG based on the evaluation of laboratory
results.
6. Approves measures to address water quality issues.
7. Reviews and approves location of water sampling points.
8. Approves the annual water quality surveillance plan of water districts/water providers.
9. Analyzes quarterly Water Quality Surveillance Reports to determine trends in the quality of
the drinking water supply service for appropriate action.
10. Issues pronouncements on the quality of drinking water in the locality and releases
corresponding advisories if necessary.
11. Conducts water quality audit whenever necessary.
12. Conducts regular quarterly meeting or as the need arises.

B. Specific Functions.
Municipal Mayor
1. Acts as chair of the Committee
2. Presides quarterly meetings or every committee meeting.
3. Signs the approved committee policies and resolutions.
4. Signs the approved annual work and financial plan
5. Delegates other related function.

Municipal Health Officer


1. Takes the over all lead in the implementation of drinking water quality surveillance program.
2. Ensures inter-sectoral linkages in the drinking water quality surveillance program
3. Convenes Committee meetings.
4. Signs communications and reports for the Committee
5. Acts as chairperson of the Committee in the absence of the city/municipal mayor
6. Designates who will take minutes of the Committee meetings.
7. Performs other related tasks

DOH -DMO Representative


1. Represents the DOH in the committee
2. Advocates/promotes DOH policies/programs/projects relative to drinking water quality
surveillance

Municipal Engineer
1. Informs the Committee on relevant civil works in the area that may affect the quality of water
sources.
2. Provides technical and logistic assistance to the committee

Municipal Agriculture Officer


1. Informs the Committee on agricultural wastes, fertilizers and pesticides that may affect the
quality of water sources.
2. Provides technical and logistic assistance to the committee

MENRO
1. Provides information to the Committee regarding incidence or occurrence of water.
contaminants arising from the domestic, industrial, commercial or institutional facilities.
2. Provides logistics and technical assistance to the committee.

CHAIR ON HEALTH AND SANITATION SANGGUNIAN BAYAN


1. Introduces appropriate resolutions/ordinances concerning drinking water quality surveillance.
2. Provide information to the Committee about existing resolutions that may affect water quality
surveillance.
3. Endorses the work and financial plan prepared by the committee for corresponding budget
appropriation by the municipal council

MUNICIPAL PLANNING DEVELOPMENT OFFICER


1. Integrates the Committee’s work and financial plan to the over-all municipal development
plan.
2. Informs the Committee on the short and long-term development plan of the municipality that
will affect water quality
3. Provides technical and logistics assistance to the committee

MANAGER OF WATER DISTRICT


1. Provides updates on water quality monitoring plan and its implementation.
2. Provides technical and logistics assistance to the committee.

NGO
1. Provides technical and logistics assistance to the committee.
2. Conducts information dissemination on drinking water quality

Municipal Sanitary Inspector


1. Provides technical and logistics assistance to the committee

Representative from Water Laboratory


1. Provides technical and logistics assistance to the committee

Section 4. Creation of TWG. The Technical Working Group shall be created also to assist the
LDWQMC and shall composed of the following:

Section 5. Functions . The TWG shall have the following functions:


A. Main Functions.
1) Conducts inventory of water supply facilities.
2) Prepares annual work and financial plan for drinking water quality surveillance.
3) Reviews and recommends approval of the annual water quality surveillance plan of water
districts/water providers.
4) Conducts sanitary survey as required per water system.
5) Collects water samples from sampling points for analysis.
6) Informs the water provider on the results of water quality monitoring.
7) Evaluates laboratory water analysis results as to compliance with PNSDW.
8) Recommends appropriate measures to correct deficiency in water quality.
9) Disseminates pronouncements and advisories issued by the committee.
10) Serves as secretariat to the committee
11) Prepares required reports by the Committee
12) Conducts monthly meeting or as the need arises.
13) Performs such other functions as may be delegated by the committee.
B. Specific Functions.
Municipal Health Officer
1. Acts as the Team Leader of the TWG
2. Convenes TWG meetings
3. Delegates administrative and technical functions
4. Designates secretary to the Committee

Sanitation Inspector
1. Leads in the conduct of drinking water quality surveillance activities (i.e. sampling, sanitary
survey, etc).
2. Keeps report and records of the TWG and Committee.
3. Performs other functions as directed by the team leader.

Representative from Municipal Engineering Office


1. Assists in the conduct of sanitary survey.
2. Assists in the review and evaluation of sanitary survey and water quality monitoring plans
3. Performs other functions as directed by the team leader

Representative from the Municipal Planning and Development Office


1. Assists in the review and evaluation of sanitary survey and water quality monitoring plans.
2. Performs other functions as directed by the team leader.

Section 6. Meetings. The meeting of the LDWQMC shall be held quarterly or as often as
necessary.

Section 7. Secretariat. The Rural Sanitary Inspector shall act as the secretariat of the LDWQMC
and shall be responsible for providing technical support, documentation of proceedings,
preparation of reports and such other assistance as may be required.

Section 8. Effectivity. This Order shall take effect immediately.

DONE THIS 24th day of July 2023 at the Office of the Municipal Mayor, Municipal Hall,
Poblacion, Sta. Josefa, Agusan del Sur Philippines.
