3. The Office shall adopt an open ranking system based on the guidelines and
criteria set forth in DepEd Order No. 007 s. 2023, Guidelines on Recruitment,
Selection, and Appointment in the Department of Education. The applicants are
highly encouraged to read the guidelines for complete understanding of the criteria and
process.

5. Applicants may submit their application documents at the SDO Receiving


Section–addressed to the Schools Division Superintendent, Attention: HRMO. Filing of
application may also be done online by sending the PDF file of their documents (basic
and criteria for evaluation) at personnel.camsur@deped.gov.ph with NAME and
POSITION APPLIED FOR as the subject of email. However, they are still required to
submit the hard copies of their documents on or before the deadline of submission to
be included in the pool of official applicants. Enclosure 1 contains the Documents for
Submission and Special Instructions.

6. No additional documents will be accepted after the deadline of submission on


July 18, 2023, 5:00 P.M. as reflected in Enclosure 2, Schedule of Activities and List
of Vacancies.

7. The applicant assumes full responsibility and accountability for the


completeness, authenticity and veracity of the documents submitted, as evidenced by
the Omnibus Sworn Statement, duly signed by the applicant. Any false and fraudulent
document submitted shall be grounds for disqualification.

8. The application code in Enclosure No. 3, Checklist of Requirements and


Omnibus Sworn Statement on the Certification on the Authenticity and Veracity
(CAV) of the documents submitted and Data Privacy Consent Form, shall be
assigned by the HRMO to all official applicants to ensure the integrity and objectivity of
the recruitment process and to protect the identity of the applicants upon posting of
results. Applicants shall be informed of their assigned application codes through e-mail
or text messages.

9. This agency upholds Equal Employment Opportunity Principle, “that there shall

DOCUMENTS FOR SUBMISSION AND SPECIAL INSTRUCTIONS

A. Basic Requirements.

These are documents required to determine the qualification of the applicant for
inclusion in the deliberation and ranking. INCOMPLETE SUBMISSION will render the
applicant disqualified to proceed with the process. Disqualified applicants will be
informed through a text message by the HRMPSB Secretariat.
1. Checklist of Requirements and Omnibus Sworn Statement on the
Certification on the Authenticity and Veracity (CAV) of the documents
submitted and Data Privacy Consent Form (found in Enclosure No. 3)
2. Letter of intent addressed to the Head of Office
3. Duly accomplished Personal Data Sheet (PDS)
4. (CS Form No. 212, Revised 2017) and Work Experience Sheet, if applicable
5. Photocopy of valid and updated PRC License/ ID, if applicable
6. Photocopy of Certificate of Eligibility/ Report of Rating, if applicable
7. Photocopy of scholastic/ academic record such as but not limited to
Transcript for Records (TOR) and Diploma, including completion of
graduate and post-graduate units/ degrees, if available
8. Photocopy of Certificate/s of Training, if applicable
9. Photocopy of Certificate of Employment, Contract of Service, or duly signed
Service Record, whichever is/ are applicable
10. Photocopy of latest appointment, if applicable
11. Photocopy of Performance Ratings in the last rating period(s) covering one
(1) year performance prior to the deadline of submission, if applicable
 Photocopy of Performance Rating obtained from the relevant work
experience, if performance rating in Item (11) is not relevant to the

shall be required
 Action Plan/ Re-entry Action Plan (REAP)/ Job Embedded
Learning (JEL)/ Impact Project applying the learnings from the
L&D intervention done/attended, duly approved by the Head of
Office;
 Accomplishment Report together with a General Certification
that the L&D intervention was used/adopted by a different office
at the local/higher level

Special Instructions for the Preparation and Submission of Documents

 Applicants are advised to prepare and submit their photocopied documents in


two (2) long size, ordinary tagboard folders:
Folder#1 – A. Basic Requirements
Folder#2 – B. Other documentary requirements for comparative assessment
which must be placed in a brown envelope, sealed and signed
across the flap by the applicant to secure the documents for
the evaluation proper.

 The folders and envelope must have the NAME and POSITION APPLIED FOR

the HRMPSB three (3) months after the release of the approved rank list. The
committee shall not be responsible for safekeeping of documents not retrieved
by the applicants.
