REASON FOR MODIFICATION

The consultant will be completing local travel for work outlined in the consultant
agreement, thus per diem for travel needs to be added.

MODIFICATION(S) TO CONSULTANT AGREEMENT

This Modification to the above referenced Consulting Agreement makes the following changes:1

Per diem for 7 days of travel in counties in Liberia.

- Daily per diem rate = $116,
- total per diem for all days = $812

Except as provided herein, all terms and conditions of the original Consultant Agreement remain unchanged, and in full force and
effect. The consultant’s signature on this modification indicates the Consultant’s agreement to the changes listed above including
any referenced attachments.
