SUBDIVISION SURVEY CONDUCTED BY THE DARA AT BARANGAY
BANENGBENG, SABLAN, BENGUET

Sir,
With all due respect, the undersigned Land Owner/ Representative, is requesting your respectable
office for the cancellation of Psd-03-102305 a subdivision survey project conducted by the local office of the DAR,
La Trinidad, Benguet, at Banengbeng, Sablan, Benguet, originally surveyed as Lot 792-A-2, San Rafael Bulacan, for
the following reasons as follows;

1. The aforementioned subdivision survey of the local office of the DAR thru their private land surveyor has
made an inadvertent error of subdividing the above-noted lot. That instead of an equal division of the
said lot as agreed among us, co-workers, only three lots have the same area as compared to the fourth
lot which has a bigger area.

2. Now, we the parties concerned as co-owners, have took it upon ourselves to agree for the cancellation
of the above-noted subdivision survey and arrive at a consensus to consolidate Lot 792-A-2, Sablan
Cadastre and equally subdivide the same for ourselves after being consolidated, both lots were titled by
the DAR with TCT No. T-140925, respectively

We hope and pray that this request be given favorable consideration on the matter.

CANCELLATION OF RESURVEY PLAN (RS-08-000275-D) AND
SUBDIVISION PLAN (CSD-08-006421-D)

We, ROSITA CONDOR, of legal age, Filipino, married and a resident Brgy. Silad, Villaba, Leyta,
Philippines, niece of MARCELA SANCHEZ; RAMON V. SANCHEZ, legal age, Filipino, married and a resident
of Brgy. Silad, Villaba, Leyte, Philippines, son of LUCIO SANCHEZ; MA. ELINETA ALMOROTO, legal age,
Filipino, married and a resident of Isabel, Leyte, Philippines, daughter of JOSE SANCHEZ, and ANGELITA
DUALLO CORPIZ, of legal age, Filipino, married and a resident of Brgy. Silad, Villaba, Leyte, Philippines,
daughter of CONSTANCIA DUALLO SANCHEZ, after having been dully sworn to in accordance with law, do
hereby depose and state:

1. That the private Isolated Survey embraced in Resurvey plan (RS-08-00275-D) and Subdivision Plan
(CSD-08-006421-D) were not in accordance with the Boundary Line of Cadastral Survey and Technical
Description of DENR Region 8 Technical Survey Services as well as the Property Identification Map of the
Municipal Assessor’s Office of Villaba Leyte;

2. That the area stated in both Resurvey Plan (RS-08-00275-D) and Subdivision Plan (CSD-08-006421-D)
that is 2,571 square meters is different from that stated in the following documents:

Technical Description Cadastral 2,576.00 sq.m.

Tax Declaration Cadastral 2,988.87 sq.m.

3. That the road Right of Way of the Partition subdivision plan does not connect or is not in line with the
installation or construction of the bridge built by the Barangay;

4. That due to the foregoing discrepancies, particularly the area and road right of way of the subject
property, we hereby request the cancellation of Resurvey plan (RS-08-00275-D) and Subdivision Plan
(CSD-08-006421-D);

5. That we are executing this affidavit to attest to the truth of the foregoing facts and to support our
request for the CANCELLATION of Resurvey Plan (RS-08-02275-D) and Subdivision Plan (CSD-08-
006421-D with the Department of Environment and Natural Resources (DENR), Lands Management
Service, Region VIII (8).
