    Objective 1-7: CO Rating
    Objective 8:

    note: included are attendance sheets, minutes of the meeting


    and reflection notes. 1 for every quarter
    Objective 9 – 10: CO Rating
    Objective 11:

    Note: any one of the following across 4 quarters ( ibig sabihin


    isa bawat quarter dapat meron maipapakita) Halimbawa: kung ILMP dapat meron
    during 1st, 2nd, 3rd at 4th quarter… O kaya nmn LP with index of mastery, 4 na DLL,
    isa bawat quarter may nakalagay na index of mastery
    Objective 12:

    note: kahit ano sa mga mov’s ay maaaring ihanda basta at


    meron 1 per quarter.
    Objective 13:

    note: one per quarter (naka focus and reflection sa application


    nyo po nang inyong philosophy on teaching in making a learner – centered
    discussion. In short pili kayo ng isang topic every quarter i-explain nyo po paano nyo
    ini-apply ang paniniwala ninyo sa pagtuturo upang maging learner centered ang
    inyong pagtuturo.
    Objective 14:

    note: lahat po nang ito ay kailangan maihanda kung nais na


    makakuha ng mataas na puntos
    Objective 15:

    note: kahit alin po ay maaaring ihanda, magsasama ng


    annotation or paliwanag kung paani ito nakatulong sa teaching-learning process o sa
    pagtuturo. Iindicate na ito ay nagawa sa loob ng 4 na quarter

    iniinterpret po ang result ng periodical test, gawan na lang ng focal group discussion,
    result ng discussion.
