I, the undersigned, am the owner of LUCKY D&J GROCERY STORE located at 3/F Leo’s Building 8135
Dr. A. Santos Ave., San Dionisio, Parañaque City.

The company applied for the registration of this business which I forget the date and my Cert. of
Registration was lost by my bookkeeper. I would like to request for the cessation of the business
registration because the company is no longer in operation and is already closed and terminated
since 2020.

This is due to bankruptcy brought about by the 3 years non-operation during the pandemic period
and after.

I hope you will grant my request which I would be very grateful of.
