Madam:

Please accept this letter as my official notification of my intent to retire effective

( _________________date) from DepEd Division of Tarlac Province, Tarlac National Highschool. I had
served TNHS from _________________ to _______________ of the current year, with 7 (seven) years
of government service. I have genuinely enjoyed the years with TNHS and that I am finally retiring
from my teaching career as per DepEd Order in relation to LET non-passers.

I will always consider myself as part of DepEd and sincerely appreciate that this working environment
has given me a lot of opportunities and memorable teaching experiences worth living till my golden
years.
