SUBJECT: POLICY ON SPECIMEN REJECTION FOR CLINICAL


CHEMISTRY TESTS

OBJECTIVE
 This policy aims to provide quality patient care by ensuring quality laboratory results through the
establishment of criteria for specimen rejection, in general and specifically, in the Clinical Chemistry
section.

SCOPE
 This covers a number of circumstances to which specimen may be rejected for testing in the Clinical
Chemistry section.

CRITERIA FOR SPECIMEN REJECTION


 Unlabelled sample
 Specimens which are improperly labeled
 Specimens with insufficient quantity for processing
 Specimens with no request form
 Insufficient patient information
 Sample label and patient name on the test request form do not match
 Nonfasting samples for tests that require fasting
 Overfasting/underfasting samples
 Hemolyzed specimens are not acceptable for potassium, bilirubin and most enzyme tests
 Sample collected in wrong tube/container (e.g. using the wrong preservative)
 Inadequate volume for the quantity of preservative
