Format of Internship Report

1. Title Page
a. Name of the organization.
b. Name of the internee, Student ID and batch.
c. Starting and ending dates of internship.
d. Submission date of the internship report
e. Name of the University and Department with logo.
2. Letter of Undertaking
Students are required to fill in the Letter of Undertaking and attach scanned copy after signing it.
3. Report Approval Certificate
This would be signed, by Chairperson DBME, upon satisfactory completion of student internship.
4. Scanned copy of internship certificate provided by the organization
Students will attach the certificate provided by the internship organization.
5. Acknowledgment
Student makes acknowledgement of the efforts put in to provide them this internship experience.
6. Executive Summary
This would be an overall summary of the whole report.

Properly made using MS Word ‘References’ tools,
8. List of tables
9. List of figures
10. List of abbreviations
11. Overview of the organization
Includes, but is not limited to, a brief history of the organization, vision and mission statement of the
organization/department, total number of employees, main offices, specialty of the organization,
introduction of all departments, role of your internship department within organization, and the
department organogram.
12. Learning Experiences
a. Knowledge acquired: Briefly describe the knowledge you gained through your training experience and
relate this knowledge to what you learned in specific courses at BHU.
b. Skills learned: Describe the skills and any career-specific abilities that you gained during your
internship. Discuss any of the skills that you learned as part of courses at BHU that were helpful during
the training. (Note: the skill means the ability to achieve something like reading and understanding
financial reports, analyzing problems, working in groups, etc.).
c. Observed professional attitudes and values: Describe the manners, mindsets or values that you found
and you perceive as important, in your training program, for a successful career (e.g. hard work,
dependability, honesty, etc.)
d. The most challenging task performed: Describe the best and the most challenging mission that was
assigned to you during your internship, how you performed it, and how you overcame challenges while
performing it.
13. Self-Evaluation
Analyze your experience and tell the good and the bad of it. Make constructive criticism. Perhaps you
learned something about yourself. Describe how you can use what you’ve learned about yourself. Be
specific and detailed in describing your experience.
14. Conclusion
Conclude the overall internship experience and describe how effective was this experience in
orientating you to professional biomedical engineering world.
15. Recommendations
Make any recommendations that might improve the internship experience.
16. References
Provide properly cited references and sources that you have used in writing this report.
