Exercise 1

1. What is your position onboard?


I am chief officer on board offshore vessel.

2. What type of vessel are you onboard?


Presently I am on board OSV Offshore Supply Vessel

3. Have you done work inside an enclosed space? If so, which enclosed space?
I have done work inside an enclosed space before on cargo vessel and offshore vessels. The
enclosed spaces I was working before are various cargo tanks, ballast tanks, chain lockers, void
spaces.

4. Does your vessel have a safety management plan for enclosed space entry?
Safety management plan for Enclosed space entry is part of our company ISM.

5. Which procedures and checklists do you have onboard your vessel regarding enclosed space entry?
1. Enclosed Space Entry Permit (pls see attached)
2. Generic Risk Assessment for entering Enclosed Spaces (pls see attached)
3. Entry into and Rescue from Enclosed Spaces Emergency Check List (pls see attached)
4. Training manual for Enclosed Space Entry

6. If you shall enter a cargo or fuel tank, why is it essential to look at the material safety data sheet
(MSDS) for the last substance in the tank before you start planning for a safe entry into the tank?
It is essential to look the MSDS sheets in order to assess any possible risks that may exist
because of residues of that substances.

7. Many accidents in enclosed spaces are caused due to underestimation of the danger or hazards, is
this a focus on onboard your vessel?
Yes on boar our vessel we are kipping that in focus, plenty incidents investigation are provided on
board by our HSEQ department.

8. Describe the safety culture in your company.


The safety culture in our company are maintained on high level, any crew member can stop any
job, if he is thinking, that is not safety, the safety officer present on board, perform regularly safety
meetings, the HSEQ department doing internal audits and spot checks.

9. How often are enclosed space entries and rescue drills executed onboard your vessel?
Enclosed space entries and rescue drills are carried out monthly basis as per SMS.

10. What is the procedure if you have personnel from shore working in any of the vessels enclosed
spaces?
To check if person from shore is qualified to enter the Enclose Space, plus same procedures
should apply, as it will be a crewmember.

11. Tendency to trust physical senses, underestimate the danger and complacent attitude are all common
factors in fatal accidents, describe how you think this factors can be avoided.
Good training of personal, use proper and approved equipment, following procedures, control on
all levels (on board and from company)
Exercise 2
1. Before you plan the exercise you should have a main goal(s) for the exercise, what will be your main
goal(s) for this exercise?
My goals are:
- I have experience in planning Enclosed Space Entry
- I familiar with on board procedures for Enclosed Space Entry
- I completed CBTs 0285 Enclosed Space Entry, Awareness; 0425 Gas measurement, a safe
atmosphere; 0426 Gas measurement, Measuring instruments.
- I read a workbook for Enclosed Space Entry course.

2. Which enclosed space did you choose?


I choose Fresh water tank no.1

3. What potential hazards or dangers and can you expect in the enclosed space you chose?
- Lack of oxygen
- Presence of Explosive atmosphere
- Presence of Toxic gases (H2S)
- Presence of Carbon dioxide (CO2)
- Risk of being trapped
- Slip, trips, falls hazard
- Risk Space being filled
- Heat stress
- Contamination chemical hazards

4. What can be the reason for these hazards or danger?


- Lack of oxygen
Not sufficient ventilation of the tank, corrosion

- Presence of Explosive atmosphere


Possible fumes of fuel or oil products

- Presence of Toxic gases (H2S)


Can be generated if oil or carbohydrates present in bilges

- Presence of Carbon dioxide (CO2)


CO2 is heavier than air, bilges are usually at the bottom, can accumulate or be generated.

- Risk of being trapped


Bilge tanks are usually small compartment, with difficult access, it is a possibility to being trapped,
if you are fat or big, or using air cylinders at your back.

- Slip, trips, falls hazard


Always present such risk, because of wet /oily surface, not sufficient illumination.

- Risk Space being filled


If somebody who are not noticed that people are in tank, will decide to transfer content of other
bilge to mentioned bilge.

- Heat stress
In case of excessive temperature in the tank, especially when it is in E/R.
- Contamination chemical hazards
Possible content of various chemicals in bilges, that may be irritant to skin, or can cause other
dangers to the health in case of contamination.

5. How can you ensure the safest possible atmosphere before entering the enclosed space?
With proper ventilation of the tank, after that will perform measurements of O2, CO2, H2S,
flammable gases on 3 levels of the tank (top, middle and bottom)

6. Which measuring instruments would you use for testing the atmosphere in the space?
I will use multi gas detector (on board we have 2 pcs Drager X-am 2500)

7. Which personal protective and safety equipment will be used in the exercise?
During exercise following Protective and Safety equipment will be used :coverall, safety
shoes/rubber boots, helmet, gloves, safety goggles (if no use air cylinders)), safety harnesses
with life-lines, gas proof torch light, personal multi gas detector, gas proof radio.

8. How would you ensure that all crew in the exercise understand the importance of the use of the
protective and safety equipment?
Before performing an exercise a tool box talk will be carried out with all Crew regarding entry to
enclosed spaces, re assess the possible risks (complete RA) including LMRA – Last minute risk
assessment.

9. Which checklists and procedures do you have to help you with in the preparation of the enclosed space
exercise?
We have on board following documents to help me with preparation of enclosed space exercise:
- Generic Risk Assessment for entering Enclosed Spaces
- Enclose space entry permit
- Entry into and Rescue from Enclosed Spaces Emergency Check List - Training manual for
Enclosed Space Entry
- Isolation certificate form
- Summary of Permits in force form

10. What communication equipment will be used during the exercise? What will be the routine for
communication?
The 2 way walky-talky (gas proof), will be agreed emergency communication procedure (by
pooling the life line), The spot radio checks will perform regularly.

11. In the exercise you must set up a rescue team, who will be in the rescue team? How would you train
the rescue team?
Only trained personnel will be in rescue team, as per company policy, a rescue team perform
rescue drill every month with simulation of rescue persons from different enclosed spaces. CBTs
0285 Enclosed Space Entry, Awareness; 0425 Gas measurement, a safe atmosphere; 0426 Gas
measurement, Measuring instruments to be completed by rescue team members.

12. What type of rescue equipment is available onboard your vessel?

Harnesses, lifelines, Tripod with hoist, stretchers, air cylinders, oxygen portable cylinders with
face mask, first aid kit.
