to participate in the leadership/awarding ceremony event entitled "SIBOL: Minimizing Risks,
Maximizing Opportunitiesy" at the Ugnayang La Salle, De La Salle University-Dasmariñas on July 11,
2023 (8:00 am to 5:00pm) subject to all the requirements of all the schools for F2F participation.

I am aware that there is/are faculty member/s who will accompany the students and who will take
all the necessary precautions to keep them safe. Nonetheless, given all efforts, reminders and warnings by
teachers and school authorities, I know that fortuitous or unexpected incident(s) beyond control of the
teachers and school authorities may still occur.
