Subject: FIELD TRIP WAIVER

This is to request your office that I be allowed to join a TALK & TOUR day educational
fieldtrip to visit the FEU Alabang on __NOVEMBER 28, 2023__ to handle by ___AR. SHAIRA
REYES_ and assisted by the ___ professors. Accordingly, I will strictly observe the Guidelines for
this educational field trip as attached herewith.

I have no objection to paying the field trip fee of __N/A___ only to be paid to the FEU Cashier. This
amount covers the air-conditioned bus fare, lunch, tour handouts, tour guide fee and others as
specified below:

Adviser and one your personal copy.

1. This is an educational field trip, an academe activity. Therefore, all rules and regulations in the
FEU Student Handbook will apply for this.
2. The bus will be waiting in front of FEU along Nicanor Reyes St. Bus leaves exactly at ______AM
and those who are not able to join for whatever reason (tardiness or health) will not be able to
refund the fee paid. The expected time of arrival at Nicanor Reyes St., Far Eastern University,
Manila is at _______PM.
3. Get the number of the bus assigned to your class. Mr./Ms./Arch.
_____________________________ will monitor/supervise all participants.
4. You are expected to behave properly. No shouting and talking aloud in the bus and while on tour.
5. It is preferable to wear your school uniform. Those who come in civilian attire must be decently
dressed, thus, no shorts or long shorts, sleeveless shirts and slippers, sandals. You must wear
tour FEU ID during the entire field trip.
6. The student may bring their _____notebook, papers, ball pen or pencil. Electronic gadgets such
as tape recorder, video camera recorder and camera (ordinary or digital) to record related
activities maybe brought at your own risk. Please watch over your belongings.
7. Snacks and drinks may be consumed in the bus. Wrapped of food and soft drink cans must be
deposited in the waste cans inside the bus.
8. Chewing gum and smoking are strictly prohibited whether in the bus or outside.
9. _____________________________will supply medicines and first aid kits in the bus it would be
best that the student brings medicine for stomach troubles, dizziness and provision for special
needs like a sickness bag in case you may need one.
10. _____________________________representatives will check the number of students per bus
after every stop and /or structure visited. Therefore, always be with the assigned group.
11. Undertaking unrelated activities such as but not limited to buying additional snacks or
pasalubong is prohibited if not with the permission of the advisers. Crossing streets must always
be done cautiously and under the supervision of the Adviser.
12. Always be within hearing and seeing distance of the tour guide/lecturer.
13. Your individual activity/class assessment regarding the visits shall be given by the respective
class adviser before the scheduled trip. This assessment is to be submitted the second meeting
after the trip.

reminded.
