COMPLAINT

I hereby complain above named respondent for violating our rights and interests in the following

manner;

1. That on February 8, 2022, previous foreman RENE BOY CORTEZ made an acknowledgement

receipt stating that JAYSON C. DIMAYUGA have received our partial payment for the deep

well excavation with a total amount of SEVEN HUNDRED FIFTY THOUSAND PESOS CASH

(PHP 750,000); the agreed price for the excavation was ONE MILLION TWO HUNDRED

THOUSAND PESOS CASH (PHP 1,200,000.00)

2. The respondent is a resident of Purok 5, Barangay Baclaran, Municipality of Balayan, Batangas;

3. The respondent was referred by a close friend from Balayan, Batangas named MART MIRANDA

through the cousin named CAMILLE

4. The respondent previously did work for and was highly recommended by MART MIRANDA;

5. The respondent asked for another payment of TWO HUNDRED FIFTY THOUSAND PESOS

(PHP 250,000) when we requested to continue the excavation for us to have the deep well we

paid, but instead I paid TWENTY-FIVE THOUSAND PESOS CASH (PHP 25,000) just for him

to proceed;
 6. The respondent and I agreed that he will only receive the remaining balance once deep well is

finished;

7. The respondent has not yet delivered a working deep well at this point;

 THEREFORE, I pray that the following reliefs be granted to us in accordance with law and/or

equity:

1. That respondent JAYSON C. DIMAYUGA be made to appear before the Lupong Tagapamayapa

of Brgy._____________, _____________,_____________;

2. That after notice and hearing, respondent be directed to fully refund the PHP 750,000 in cash or

check;

3. Other reliefs just and equitable are likewise prayed for.
