 TEMPERATURE
(Clean your hands first or alcohol and ready the equipment already)

Temperature = (36.5 – 37.5 normal range)


Pulse Rate = (60 to 100 beats per minute)
Respiratory Rate = (16 to 20 beat per minute) (More than 24 he/she has tachypnea
*Takupniya*)
BP = (Less than 120 and Less than 80 is normal , 120-139 and 80-89 is within the
Prehypertension Range)

 Hi! Good afternoon May I know how should I address you? Okay (Mam/Sir). I’m Kennedy and
I’ll be your caregiver for today. How are you feeling today? Can I see your arm band? Can you
tell me your name please?...... and your birthday? Okay Mam [name of the person], Imma take
your temperature now…. I’m just going to roll up your sleeves a bit so I can get your accurate
temperature …. While waiting for your temperature I’m going to take your pulse rate, can you
extend your arms please [make sure the palm is open] are you comfortable? …. [take his/her
pulse within 60 secs] …. Indicate his/her pulse rate (60 to 100 beats per min is normal) ….
Your pulse rate is [72 beats per min which is within the normal range] and your temperature is
[36.5 Celsius which is within the normal range as well] put back his/her arm are you
comfortable?

strongest pulse within the antecubital space then put your stethoscope there and being
pumping and remember the first loud sound and the last sound. REMEMBER – THE FIRST
SOUND IS THE SYSTOLIC PRESSURE AND THE LAST SOUND IS THE DIASTOLIC
PRESSURE! …. After that remove the cuff and roll down the sleeves and move his/her hands
down …. And your BP is [110/70 which is within the normal range] wash your apparatus and
hands and after that … Do you feel comfortable? Okay Good, Take care and call me if you
