SCRIPT OF MEASSURING VITAL SIGN

Greeting
Nurse :“ Assalamualaikum, good morning”
Patient :“ waalaikumsalam”
Nurse :“ im Siska, im nurse on duty today, can you mention your name please?”
Patient :“ my name is………”
Nurse :“ what is your feeling today?”
Patient :“ im not feel good enough, im feel dizzy”
Nurse :“ oke sir, i will meassuring your vital sign this morning to knowing condition
of your body, which is like your temperature, blood pressure, heart rate and
the last is respiration rate, would you?”
Patient :“ yes of course”

Procedure
Nurse :“ im sorry but for the family, please leave us for a while to do this procedure,
or just one family member can accompany the patient in this room
Sister :” oke I will accompany my brother, Ervina u can wait us in the outside”
Ervina :” oke, call me if you need something”
Nurse :“ sir I will start this procedure, please put this thermometer on your left axilla
and you have to accros your hand to the right, it is maybe take 7 till 10
minutes, do you need my help?”
Patient :“ oke, no I can do this by my self”
Nurse :“ next I want to check your blood pressure, do you mind if I menyisingkan
lengan baju bapak untuk memudahkan pengukuran tekanan darah?”
Patient :“ yes of course”
Nurse :“ now I will check your heart rate on your wrist for one minutes” ( checking
heart rate), oke the last is checking your respiration rate for a minute too, you
don’t need to do anything, just breath normally”
Patient :“ oke maam”.
Nurse :“ it is time to take thermometer on your axilla, would you help me to do
that?”
Patient :“ here maam” ( take thermometer from his axilla)

Suggestion
Nurse :“ sir I have finished all the procedure, from the measuring result, your blood
pressure is 110/70 mmHg, your heart rate is 78 X/minutes, and your
respiration rate is 20 X/ minutes, that’s all normal, but for your temperature is
38,8̊celcius, you got fever, I will report this to the doctor to get your
medication, but before that I suggest you to compress your body, to decrease
the temperature”
Patient :” thank you maam”

Asking Question
Nurse :” your welcome, any question?”
Sister :” is there any option to decrease his temperature maam?”
Nurse :” he must drink enough water, that’s good too to decrease temperature”
Sister :” thank you very much maam”
