<10kg: 200mg/dose, >10kg: 400mg/dose; or
Albendazole 200mg/dose (12 mo to 2 yr), 400mg/dose (>2 yr)
(Frequency and duration of treatment depend on types of parasite)
Amoxicillin 10-25mg/kg/dose (adult 0.25-1g) 8H; or 12.5-40mg/kg/dose 12H
Amoxicillin/Clavulanic Acid (For preparation containing 200mg/28.5mg)
12.5-22.5mg/kg/dose (amoxicillin component) 12H
Ampicillin 12.5-25mg/kg/dose (adult 0.25-1g) 6H
10-15mg/kg/dose (adult 500mg) on day 1, then 5-
Azithromycin
7.5mg/kg/dose (adult 250mg) 24H on day 2-5; or
10-15mg/kg/dose (adult 500mg) 24H for 3 days
Bromhexine 0.15-0.3mg/kg/dose (adult 8-16mg) 8H
Cefuroxime 10-15mg/kg/dose (adult 250-500mg) 12H
12.5-25mg/kg/dose (adult 0.5-1g) 6-12H;
Cephalexin
Prophylaxis: 12.5mg/kg/dose (max 250mg) at night
Chlorpheniramine 0.1mg/kg/dose (adult 4mg) 6-8H
Clarithromycin 7.5-15mg/kg/dose (adult 250-500mg) 12H
Cloxacillin 15-25mg/kg/dose (adult 500mg) 6H
Co-trimoxazole TMP 4-6mg/kg/dose (adult 80-160mg) 12H;
(Sulfamethoxazole/Trimethoprim) Prophylaxis: (Renal) TMP 2mg/kg/dose (max 80mg) 24H,
(P. Jirovecii) 5mg/kg/dose 24H on 3 days/wk
1-2mg/kg/dose (adult 50-100mg) 6-8H; or
Diphenhydramine 6.25-12.5mg (2 to <6 yr), 12.5-25mg (6 to <12 yr), 25-50mg
(≥12 yr) 6-8H
Domperidone 0.2-0.5mg/kg/dose (adult 10-20mg) 4-8H
Erythromycin Ethylsuccinate <2 months old may cause pyloric stenosis
15-25mg/kg/dose 12H; or 30-50mg/kg/DAY divided every 6-8H
Ferrous salts 1-6mg elemental iron/kg/DAY in 1-3 divided doses
Folic Acid 50mcg (neonate), 0.1-0.25mg (<4 yr), 0.5-1mg (>4yr) 24H
Lactulose 1-3ml/kg/DAY (max 60ml/DAY) in divided doses
Metoclopramide 0.1-0.3mg/kg/dose (adult 10-15mg) 8H
3mg/kg/dose (adult 75mg) 12-24H; or
For infant: 12mg (<3 mo), 20mg (3 to 5 mo), 25mg (6 to 11mo);
Oseltamivir
For children ≥1 yr: 30mg ( ≤15kg), 45mg (≥15 to ≤23kg),
60mg (>23 to ≤40kg), 75mg (>40kg) 12-24H

Salbutamol 0.1-0.15mg/kg/dose (adult 2-4mg) 6-8H
