RE: Temporary Guardianship of Rhyle Xyl E. Ababa

Dear, Jasmine DG. Enriquez

I, Jobelyn DG. Enriquez, the parent/legal guardian of Rhyle Xyl E. Ababa, born on
March 13, 2011, hereby grant temporary guardianship of my child to Jasmine DG.
Enriquez effective from 2023.

During this period, Jasmine DG. Enriquez will have the authority to make decisions
regarding my child’s health, education, and welfare.

I authorize Jasmine DG. Enriquez to seek medical treatment for my child, if necessary,
including but not limited to emergency care, hospitalization, and routine checkups.

Additionally, Jasmine DG. Enriquez may enroll my child in school, sign permission slips,
and participate in parent-teacher conferences.

This temporary guardianship agreement may be revoked or modified at any time by

providing written notice to Jasmine DG. Enriquez. Thank you for your understanding
and assistance in ensuring the well-being of my child during my absence.
