2. PROTECTING DATA COLLECTED

Access to my personal data will at all times be restricted to authorised personnel only, who will
maintain its confidentiality.
The Service Provider and Sponsor will ensure that the level of protection for my personal data
complies with applicable personal data protection regulations and that the level is equivalent to
what is required within the South Africa and the European Union.

3. CONFIDENTIALITY

Throughout the study and for a 10-year period starting at the end of the Study, I agree not to
reveal, copy, or use any information regarding the Products and the Study that I might learn about
during the course of the Study. I agree not to share any documents in my possession regarding the
Study with anyone and to destroy them promptly regardless of the type of media (i.e., paper, photo,
video, etc.).

4. FEEDBACK & SUGGESTIONS

I understand that I may be asked to make Suggestions regarding new products, devices, and/or
services in the xxx sectors and to provide Feedback on the Products and/or services that I will
test/use, I hereby cede and assign to the Sponsor all intellectual property rights regarding my
Suggestions and Feedback. The Sponsor will therefore have full ownership of the Suggestions and
Feedback without any sort of limitation.

5. CONSENT

The manner in which my personal information will be collected and processed has been set out in
clause 1 (Personal Data) above.

My supply of my personal data is voluntary and should I elect not to provide my personal data, then
in consequence thereof, I will not be able to participate in the Study.

I have read and understood this consent letter. I acknowledge that the Service Provider has
moreover clearly explained and described the Study to me in terms of its objectives, nature, length,
location, conditions for being conducted, and constraints in order to ensure that my consent to
participate in the Study and the processing of my personal data is well informed.
