MONITORING AND EVALUATION TOOL

Instruction

This CID initiated M & E tool will be used in monitoring and evaluating school science
curriculum delivery in relation to Division Memorandum No. 154, s. 2021 or Tanaw Agham
2022-2024. The components of the Tanaw Agham 2024 Pedagogical Model reflects to the
paradigm of K-12 Basic Education Curriculum Framework, K-12 Science Curriculum
Framework, Philippine Science Education Framework and lastly, Philippine Professional
Standards for Teachers. It is designed with criteria and indicators aligned to the division
pedagogical model. This tool will be used by CID personnel for quarterly monitoring of
schools. In addition, school head, master teachers, and science teachers themselves may
use this tool to score day-to-day teaching and to generate meaningful reflections that serve
as bases in crafting relevant curriculum decisions.
The data that will be successfully collected using this tool serve as source of
information in identifying “priority schools” or schools needing technical assistance and
curriculum support. In other words, the purpose of this tool is to identify the gaps of science
curriculum implementation and create interventions and relevant solutions to the identified
gaps. Basic research will also be crafted to formally document schools’ performance in terms
of science curriculum delivery. The indicators enumerated in this tool may emerge
highlighting relevance and appropriateness.
To begin with the monitoring and evaluation activities, the user should fill in with
necessary information the table below as voluntarily shared by the client subjected to data
privacy act. In scoring, each evident indicator is equivalent to one point. Interpretation of
the results will be based on the scale below:

M&E Term of Reference
City Schools Division Tanauan supports the provision of RA 10533 commonly known
as Enhanced Basic Education Act of 2013 and the guidelines stipulated in DepEd Order
No. 21 s. 2019 or Policy Guidelines on the K to 12 Basic Education Program. This tool will
be the primary source of data that will serve as bases in creating relevant programs,
projects, and activities. Thus, the following terms of reference:
Division Personnel
Chief education Supervisors, Education Program Supervisors, Public Schools
District Supervisor, and Senior Education Program Specialists/ Junior Education
Program Specialists and other non-teaching division personnel shall use the tool in
monitoring and evaluating schools in terms of science curriculum delivery.
Specifically, they are expected to:
1. accomplish the tool to gather data on science curriculum implementation
in the division.
2. utilize the tool in monitoring public elementary and secondary schools (and
private schools upon agreement).
3. suggest relevant constructs for the continuous improvement of the tool.
4. submit the data gathered to education program supervisor in charge with
the curriculum for consolidation purposes.
School Head
School Principals, Head Teachers, and Teachers-in-Charge in public
elementary and secondary schools (private schools upon agreement) shall adopt the
tool in monitoring and evaluation of the school in the implementation of science
curriculum. Particularly, they are expected to:
1. accomplish the tool to gather data on science curriculum implementation
in the school level.
2. utilize the tool in monitoring science teachers’ accomplishment on
curriculum, instruction, and assessment.
3. suggest relevant constructs for the continuous improvement of the tool.
4. relay the data gathered to division personnel monitoring/evaluating the
school or submit the gathered data to the education program supervisor in
charge with the curriculum, or Public Schools District Supervisor assigned
to supervise the said school for consolidation and TA purposes.
Science Teachers
Science teachers of both public elementary and secondary schools shall adopt
the tool for self-assessment purposes.

Effectivity
The Tanaw Agham M&E tool (localized tool) will be adopted in monitoring and
evaluating the schools’ accomplishment/endeavors in the division for the School Year
2021-2022, 2022 – 2023, and 2023 – 2024.
