1.0 PURPOSE
This Procedure specifies the nature of Serious Incidents, support processes and
responsibilities of AZN O&M employees in the event of a Serious Incident.
The document is designed to identify roles and responsibilities for staff in dealing with
emergencies, and to introduce a "live" working document for staff to provide a structured
pack of information, which can be accessed quickly in the case of a site emergency.
In the event of an emergency, all activities will be co-ordinated from the Control Room by
the Shift Charge Engineer acting as Incident Controller.
2.0 PRINCIPLE
A serious incident for the purpose of this procedure is defined as an incident that arises
due to major plant related problems, extreme health problems, natural disasters, acts of
terrorism, or sabotage that expose AZN or any person therein to serious risk.
It also applies where there is the potential for media interest or reputational risk.
All incidents/emergencies should be reported to the Control Room immediately.

The following should always be in a possession of a hard copy of this procedure:


a) General Manager
b) Operations Manager
c) Chief Finance Officer
d) Engineering Manager
e) HR & Administration Manager
f) Maintenance Manager
g) Fire Officer
h) HSE Manager
i) Deputy HSE Manager
j) Main Control Room
Any other copies are UNCONTROLLED, and as such are not formal documents.
Any reissue of this procedure must include replacing the above copies with new
versions.
3.0 SCOPE
This Emergency Procedure is to ensure that serious incidents are effectively managed
and to provide support where required.
This procedure applies to Serious Incidents concerning events, the combination of events,
or potential events that affect or may affect:
a) The safety of people;
b) Property (facilities, own buildings, third-party buildings);
c) Intangible assets, IT systems or data;
d) The reputation of the company or any of its parts
Note that for the purposes of Serious Incident reporting, AZN should be considered to be
included in the ENGIE and Sumitomo reporting structures, and any references to
Sumitomo, ENGIE, Kharabel, International Power or any other company reference should
be considered as the Parent Company.
4.0 RESPONSIBILITY
The General Manager is responsible for:
• Ensuring that sufficient resources are available to provide training and drills
for all persons in connection with this procedure

• Ensuring that all relevant personnel, including the regional office and the
Owner, are informed of any serious incident as soon as reasonably
practicable.
The duty Shift Charge Engineer, in an emergency, will:
• Ensure the plant is in a safe state and maintain responsibility for the plant and
site until relieved by another member of staff or accredited person.
• Manage the emergency, acting as Incident Controller. For ease of
identification on site the Incident Controller will wear the designated high
visibility vest which is kept in the Control Room
• Ensure individual aspects of emergency management are properly
transferred to relevant responsible persons e.g. Fire Officer, Police,
Ambulance Service, Environmental Contractor etc. at the appropriate time.
• Ensure the incident is reported promptly and accurately in accordance with
the appropriate site procedures
• Give the ‘all clear’ at the appropriate time, and inform staff and contractors
that they may return to work.
Note: No further work may be carried out under any Safety Document until a re-assessment
has been carried out by a Safety Controller, as required by the Application of Safety Rules
procedure.
Security Staff, in an emergency, will:
• Immediately open the site entrance and maintain clear access for the
emergency services.
• Maintain contact with Control Room at all times.
• Assume the duties of ROLL CALL OFFICER and account for all staff,
informing the Control Room of the result as soon as possible.
• Ensure nobody returns on site until the 'all clear' is given, unless authorised
by the Incident Coordinator or Incident Controller
• Ensure all staff log back on before they return to work.
First Aiders
• First Aiders who are not on shift shall report to an assembly point where they
may be requested to go onto site to provide first aid. They should ensure they
have the appropriate PPE for access to site.
The HR/Administration Manager will:
• Ensure that the contract Security Staff have received assignment instructions
which clearly outlines their responsibilities
• Ensuring that an up to date staff contact list is available
The HSE Manager is responsible for:
• Ensuring that location based procedures are in place to deal with any Serious
Incident.
• Ensuring that an up to date contact list is available for communication with all
relevant persons in the event of a Serious Incident
• Ensuring Serious Incident training is given to all relevant staff.
The Fire Officer is responsible for:
• Providing expertise in managing an incident and in managing test drills.
• Training and drilling the Emergency Response Team
• Maintaining the site emergency vehicles
• Ensuring that the requirements of this procedure are tested as frequently as
is determined by site emergency procedures.

• Ensuring that any lessons learned from exercises are fed back into site
procedures and processes as required.
Operations and Maintenance Managers are responsible for:
• Ensure full compliance with the requirements of this procedure.
• Implement controls of this procedure.
All Station Staff will:
• Ensure that they have read and understood the detail of this instruction.
• Ensure when acting as Technical Officers that their contractors and visitors
are aware of the implications of this instruction having been briefed during site
induction.
When hosting visitors on site will:
• Ensure their visitors undergo sufficient induction to be familiar with the site
alarms and actions in event of hearing them
• Ensure that they are familiar with the emergency procedures and be
responsible for their visitors’ health, safety and welfare whilst on site. On
hearing the evacuation alarm, staff with visitors will guide them by a safe route
to an assembly point, where they will remain until accounted for and the ‘all
clear’ is given.
5.0 DEFINITIONS/ ABBREVIATIONS
A Serious Incident may relate to one of the following examples:
• Health and safety incidents;
• Environmental incidents;
• Catastrophic plant failures;
• Threat to public health;
• Damage to public or third party property;
• Political / civil unrest;
• Natural disasters;
• Acts of terrorism;
• Sabotage

A serious incident must be managed effectively to ensure that any injuries, plant damage
or adverse publicity are minimised.
8.0 PROCEDURE
8.1 Site access and emergency response accounting
All staff, visitors and contractors to receive entry passes (with photograph where
practicable) or record their presence on site by using the fingerprint scanner
A bus driver may collect all passes from gatehouse, if it has regular passengers, and give
to individuals if required
Site Security maintain list of all present on site
All visitors and contractors leave contact details and their point of contact whilst on site
with Site Security
All persons to keep pass with them at all times whilst on site

All staff, visitors and contractors to undergo site induction prior to working on site, to be
delivered by Site Security, Admin or HSE
All staff, visitors and contractors to receive copy of site induction booklet, including
• Emergency number for site
• Other important phone numbers
• Simple plan of the site showing assembly points
• Summary of PPE rules
• Summary of site HSE requirements
Evacuation assembly points are sited near to Seawater Intake (towards the Hypochlorite
Generation Building), outside the Central Control Building and in front of the
Administration Building.
In event of a site evacuation, the first member of staff to arrive at each assembly point
takes charge of the assembly point and lists all persons arriving.
When they arrive the person in charge contacts Site Security to inform them that they are
in charge of the assembly point.
The person in charge contacts Site Security with the list of people accounted for. This is
repeated as required until all persons are accounted for.
Site Security combines the 3 lists and compares to list of persons on site, which is kept
up to date by Site Security, based on the site passes in use and the database of fingerprint
scans. This list is also available in the Central Control Room.
Site Security provide a final list as soon as possible to Incident Controller to account for
all persons
Site Security to contact MEW security if emergency vehicles have been called to ensure
they are allowed site access.
8.2 All Emergency situations are dealt with by the Control Room
Telephones are situated around the site and should an emergency occur then the control
room should be called by dialling 5555
Inform the control room of the following:
• Your name
• Your location and location of the incident if different from yours
• Type of incident, Fire, Spill etc.
• Number and type of Casualties, if any;
and await further instructions.
If you are in the location of an incident when it occurs, assist only if it is safe to do so and
you know what you are doing. If in doubt, contact the Control Room.
Only attempt to tackle the incident after you have informed the Control Room and it is
safe to do so.
8.3 Fire Alarm
Local Fire alarm: will be sounded automatically by the fire detection system or a manual
call point; the local fire alarm is then investigated by a Field Operator, who will ensure that
the central fire alarm is sounded if required. If in the area, evacuate to a safe place until
the all clear is given. If unsure, move to the nearest assembly point
The duty Shift Charge Engineer shall
• Investigate the cause of the alarm
• Confirm if genuine
• Initiate Evacuation alarm if necessary
• Call Emergency Services on 112 if required

The site Fire Officer should always be informed of any fire alarm, whether confirmed as
genuine or believed to be a ‘false’ alarm.
8.4 Evacuation Alarm
Evacuation alarm: Initiated at the discretion of the Shift Charge Engineer
A site evacuation will be initiated by an alarm. ALL PERSONNEL, apart from those with
specific responsibilities under this procedure, will go to an Assembly Point where they will
register their attendance and await further instructions.
The public address (PA) announcement may direct staff by a specific route to avoid any
hazard; you must comply with these instructions.
8.5 Medical emergencies
In the event of any medical or first aid requirements, the Shift Charge Engineer should
contact the site Occupational Health Nurse in the first instance.

If the OHN is not available, Az Zour South may be available to offer emergency
treatment. Their facilities are limited but they have an ambulance available and may be
able to assist.

If any situation is life threatening you must call for an official ambulance on 112 in
case AZS are not available.

8.6 Evacuation / Fire Alarm Test:


This test occurs each week at a set day and time. Before the test an announcement that
a test is to occur is broadcast and again upon completion of the test. During the test, there
is no need to respond.
8.7 Assembly Point
There are three assembly points, identified by a white pictogram on a green background.
These are located:
NEAR ADMINISTRATION BUILDING CAR PARK
NEAR CCR BUILDING CAR PARK and
NEAR SEAWATER INTAKE AREA
Unless given other instructions over the PA system the following basic rules should be
followed:
Proceed to the assembly point and ensure that the person taking the roll call records your
name. This is YOUR responsibility. If you are the first person to arrive, follow the
requirements of section 8.1.
8.8 Recording the Emergency
The Control Room staff will record details in the Station Log and will advise the person
reporting the incident of the action to be taken until the arrival of the Fire Team or other
assistance.
The person in the CONTROL ROOM receiving the call will log the following:
• TIME of call
• NAME of the person reporting the incident
• LOCATION of the incident
• Type of incident e.g. Fire, Spill etc.
• Number and type of Casualties if any.

The SHIFT CHARGE ENGINEER will initiate the appropriate procedure and mobilise the
EMERGENCY RESPONSE TEAM if required.

Appendix 1 Actions in event of serious gas leak

1. Action on Discovering a Gas Leak


The person discovering the leak should move to a safe, well ventilated area and inform the
control room by dialling 5555 and follow any instructions given.

2. Action on Receiving a Report of a Gas Leak


The person receiving the call should ascertain and log the following:

Time of call
Name of person reporting the leak
Location of the leak plus estimate of severity
Number of casualties if any

The person receiving the call should carry out the following actions:

Inform the Incident Controller of the leak


Use Public Address (PA) system to stop all hot work, grinding etc.
Mobilise the site Fire Team
Sound the station evacuation alarm informing the staff of any hazards using the PA
system if required
Summon the emergency services if required

3. Shift Charge Engineer


On being informed of the gas leak, the Shift Charge Engineer shall:

Discuss with the control room operator to ascertain details of the gas leak.
Investigate gas leak if appropriate and establish gas to air mixture using an approved
gas tester
Decide whether to shut down plant and evacuate control room
Instigate gas emergency shutdown procedure if required.
Isolate and make safe plant as necessary.
Delegate person to meet at the site entrance and act as guide for emergency services
if required
Assume roll of INCIDENT CONTROLLER and the hi-viz vest for easy recognition.

4. Field Operators
On hearing the evacuation alarm, the Field Operators will contact the control room for
instructions.

5. Incident Controller (Shift Charge Engineer or delegate)


Don the Incident Controllers Bib for ease of recognition.
Establish communications with the Control Room.
Inform General Manager and Operations Manager if incident is out of hours.
Liaise with Emergency Services and Fire Team
Authorise "All Clear" on instruction from Emergency Services

6. Security Staff
On being informed of a gas incident by the control room:
Stop any traffic or persons from entering site
Maintain clear access to site for the Emergency Services
On sounding of the Evacuation Alarm, ensure all staff, contractors and visitors are accounted
for
Ensure no one leaves site until released by the Incident Controller.
Perform duties of ROLL OFFICER

7. All Other Staff


On sounding of the evacuation alarm, personnel without specific duties during the incident
shall report to the assembly point by the route directed by control room and make sure they
are accounted for to the security staff. Staff may not leave the assembly point until the all clear
is sounded or authorised by the Incident Controller following discussion with emergency
services or site Fire Team.

Staff meeting visitors on site shall ensure that they are familiar with the emergency procedures
and will be responsible for their Health, Safety and Welfare whilst on site. On hearing the
evacuation alarm, staff with visitors shall guide them in an orderly fashion by a safe route to
the assembly point, where they will remain until accounted for and the "all clear" is given.

8. Gas Detection – LEL% & Actions


The Incident Controller will ensure the following action is complied with according to the
severity of LEL.

LEL Action
0 to 20% Barrier off and ventilate affected areas. No naked flames, welding or grinding
to take place within affected area.
20% to 70% Evacuate and ventilate affected areas. Barrier off areas and ensure only entry
is given by a person competent to rectify faults accompanied by a safety
controller.
No sources of ignition and minimal electrical switching in that area.
70% to 90% Evacuate the affected areas immediately and shut down the affected plant as
soon as practicable.
Ensure no sources of ignition and prevent any electrical switching in that
area.
> 90% Evacuate the station immediately and shut down the station when all
personnel are clear of the affected area.
Do not return into the area until LEL levels have fallen below 70%.

Appendix 2 Actions in event of fire

1. Action on Receipt of a Fire Alarm


If a fire alarm is activated:

The alarm may not be accepted on the fire panel until it has been investigated and its status
confirmed.

All personnel (except the Duty Shift Team) should evacuate the building and assemble at a
safe distance away from the building until either the ALL CLEAR is given to return to the
building or the evacuation alarm is sounded for all personnel to evacuate the site.

The person manning the Control Room should contact a member of the shift team who will
investigate the alarm taking care not to endanger themselves, and report their findings to the
Control Room.

On confirmation of a fire, the Control Room operator should enact the following procedure:

• Location, size and type of fire
• Number of casualties if any

Inform the Shift Charge Engineer of the location and severity of the fire

Summon the emergency services (Fire Service / Ambulance) if required

Advise Fire Service of site hazards such as HV and LV supplies and chemical hazards
Inform Fire Service of any missing persons.
Authorise "All Clear" on instruction from Fire Service.

7. Security Staff
On being informed of a Fire by the Control Room and sounding of the Evacuation Alarm

Stop any traffic or persons from entering site


Maintain clear access to site for the Fire Service
Contact security staff at the project gate and ensure access is allowed for emergency services
Ensure no one leaves the assembly point until released by the Incident Controller.
Perform duties of ROLL OFFICER

Appendix 3 Actions in event of bomb threat

1. Introduction
The actual response to an incident will depend to a large degree on the circumstances, and
some flexibility must be allowed to enable the Incident Controller to make decisions that they
believe are most appropriate to ensure the safety of staff, contractors and visitors.

All staff should be aware of the possibility of receiving a bomb, through either the post or left
by a visitor. Staff should be vigilant, and watch out for unusual mail or parcel deliveries,
suspicious packages, or unauthorised vehicles in car parks or on site.

2. On Receipt of a Bomb Threat by Telephone


The person receiving the call should endeavour to keep the caller talking and record the
following:
i) Time of call
ii) The exact contents of the message
iii) The caller’s number if displayed

The person receiving the call should attempt to ascertain the following:

• The caller’s phone number


• When is the device due to explode?
• What is the location of the device?
• What type of device is it Incendiary/Fire Bomb/Explosive?
• How big is the device?
• When was it planted?
• Why have we been chosen for this action?
• Do they realise that people could be injured or killed?

Contact the Shift Charge Engineer and inform him of the call and supply him with all the
information.

The Shift Charge Engineer will assume the role of Incident Controller then direct the following:
• Searching of the control room, evacuation routes and the assembly point.
• Mobilising of the Emergency Response Team
• Contacting the police on 112
• Evacuation of the site (whilst considering that a bomb may have been
planted near to an assembly point)
• Informing the General Manager of the situation, who will ensure that any
further reporting is carried out

Once general staff safety is established, and with the agreement of the emergency services,
the Incident Controller should consider arranging for a search to be carried out of the whole
site. CCTV cameras may be used for an initial search. All members of the search team must
have a means of communicating with the Control Room or the Incident Controller and a log of
who is searching where must be maintained.
The responsibility for arranging a search of the site may rest with the site occupiers, i.e. staff
members. The emergency services may assist or give advice if requested to do so. If a suspect
device is found the police will then take control of the situation.

TOUCHED, APPROACHED OR MOVED. WITHDRAW FROM THE VICINITY AND INFORM
THE INCIDENT CONTROLLER.
DO NOT USE RADIOS NEAR A SUSPECTED BOMB

Each searcher shall be given explicit instructions as to which area to search and will be issued
with a copy of the guidelines below. They must not disturb any suspected bomb.

If a suspected bomb is found the Incident Controller will ask the Police for further advice. Any
areas where a device is suspected or detonated must as far as possible be cordoned off to
preserve evidence in the event of any criminal proceedings.

If the Incident Controller or the emergency services believe the threat to be genuine, the
Incident Controller will inform the senior member of staff, who will co-ordinate the evacuation
of the muster point to a safe area. This may be at an alternative safe area.

The Incident Controller, on advice from the emergency services, may decide to evacuate the
control room, after first shutting down the plant and 400 kV electrical systems. Should this be
the case he will retain a copy of the emergency procedures for reference.

Control of the site should then be put in the hands of the controlling officer of the emergency
services, normally the senior police officer at the scene.

The Incident Controller should remain available at all times during the incident to provide
information to the emergency services. In some circumstances, this may entail handing
responsibilities to the next shift, but only by agreement with the emergency services.

Should an explosion occur the Incident Controller must ensure that no damage to the plant
has or is likely to occur that may safely be prevented. Any damage that may cause
environmental damage must be reported to the Owner.

The Incident Controller will only make the decision to return staff to site on advice from the
controlling officer of the emergency services.

NOTES FOR GUIDANCE OF CONTROL ROOM AND DAY STAFF

1. A copy of the checklist should be kept handy at all times.

2. On receiving a bomb threat call, remain calm and courteous and be quick thinking. It
may be possible to gain more information than would at first appear likely. By engaging
the caller in conversations or by pretending not to understand (e.g. "I am sorry would
you repeat that bit" - or - "I am sorry but it seems to be a bad line, I missed that last
bit") the call may be extended.

3. Do not interrupt the caller.

4. Do not put the call through to an extension (the call may be lost due to mechanical
failure, non-attendance at the extension or the caller may refuse to wait). This does
not apply should the call come from the Police who may well ask to be put through to
a senior member of staff.

5. If possible get another person to listen in.

6. Fill the checklist as soon as possible.

WHAT TO LOOK FOR

The present day bomb can be an everyday article so that the task of identifying anything which
'does not belong' becomes quite complicated. It could be contained in or made to resemble a
carton, shopping bag, briefcase, biscuit tin, vacuum flask or parcel and will not normally arouse
suspicion. Plastic explosive on the other hand can be moulded like clay and can be placed in
position on machinery, plant or equipment without any need for a container.

Many of these bombs rely on a small electric battery to ignite the explosive and the methods
used to complete the circuit can result in instantaneous explosion should the device be
handled, or a delay of several hours if left undisturbed.

There are other ways of setting off the devices but it is most important to remember it is always
possible and comparatively simple to include a second fuse that will set the device off should
it be disturbed.

THE SEARCH

Bearing in mind the above information make a visual search only.

DO NOT TOUCH, PICK UP OR DISTURB ANYTHING YOU CANNOT READILY IDENTIFY


BUT REPORT THE MATTER TO THE CONTROL POINT IMMEDIATELY.

An initial search will be to ensure that evacuation routes are clear. Report to Control Room
when the search has been completed.

Particular attention should be paid to cableways, lavatories (including cisterns), cloakrooms,


walkways and stairways.

Do not forget to look up above eye level, behind wall radiators, behind firefighting appliances,
litter bins, dust bins lids and lift shafts, under cushions of chairs in reception areas or even on
window sills.

REMEMBER DO NOT TOUCH OR DISTURB ANYTHING SUSPICIOUS.

The Police will arrange for experts to examine and dispose of suspected bombs.

PACKAGE BOMBS

The following information may be of use when dealing with suspected package bombs.

Postal bombs are not likely to be in large parcels, but rather in flat letters weighing up to four
ounces, or in packets the size of a book. Bombs can however, be of any shape or size and
anything suspicious should be reported. They could be delivered by unwitting visitor or parcel
delivery company.

REMEMBER they may explode on opening, so look out for:

• The postmark and any name and address of sender

• The balance which if uneven is suspect

• The weight; if it seems excessive for size, treat as suspect

• The feel, which if springy or stiff with cardboard may indicate a trap

• Any protruding wires (even the best prepared devices can go adrift in transit) treat as
suspect

• Any holes (like pin holes) in wrapping or envelope

• Greasy marks on wrapping or envelope, which could be from sweating explosives

• The smell, some explosives have an aroma of marzipan or almonds

• The booby trap could be one envelope tightly taped or tied with string inside another
examine both for indications listed above

CHECK ALL MAIL CAREFULLY

1. Introduction
It is important to ensure that any report to the Environment Protection Authority, verbally or in
writing, does not admit a breach of the licence or liability for any incident. This may only be
done by the General Manager and must be via the Owner.

Advice on neutralisation and disposal of the spillage of hazardous substances should be


sought from the Shift Chemist.

2. Scope
All unauthorised discharges and exceedance of licensed limits of discharges to air, land or
water from AZN.

3. Environmental Incidents
(Including those involving a breach or suspected breach of the environmental licence limits or
conditions)

Investigate the plant status and carry out the appropriate corrective actions to alleviate the
cause of the incident.

Confirm breach conditions by manual grab samples etc., if practicable.

IF THE INCIDENT IS MAJOR OR HAS THE POSSIBILITY OF PUTTING THE PUBLIC AT


DANGER, THE GENERAL MANAGER, HSE MANAGER OR OPERATIONS MANAGER
SHOULD BE INFORMED IMMEDIATELY.

Ensure that other staff are informed via the shift log to prevent a recurrence of the incident.

4. Reporting
An Incident Report should be initiated via Intelex giving as much information as possible about
the incident (i.e. date, time, location, plant status, quantity and duration) and the corrective
actions taken. Note that the incident report may constitute evidence, and care should be taken
with the wording of the report.

In the event of an incident that meets any of the conditions in section 2 above, KEPA should
be informed as soon as practicable via the Owner after informing the General Manager or
HSE Manager.

5. Procedure to be adopted in the event of a major spillage


Examples of what can constitute a major spillage include:
• Release of chemicals posing an immediate risk to health or to the environment
• Sudden release of oil or chemical caused by plant failure or rupture (whether contained
within a bund or not)
• Leaked release of oil or chemical which is uncontained and risks contaminating the
soil, atmosphere, drains or has been released off site or is in danger of doing so

The person discovering the spillage should isolate the cause of the spillage only if safe to do
so. If not they should evacuate the affected area and inform the control room on 5555
If a spillage of a hazardous chemical is discovered, assistance should be sought from the site
Fire Team or the emergency services. Access to the area must be prohibited unless wearing
breathing apparatus.

6. Action on Receiving a Report of a Spillage


The person receiving the call should ascertain and log the following:

Time of call
Name of person reporting the leak
Location of the spillage
Substance involved if known
Extent of spillage
Number of casualties if any

They should also inform the Control Room Operator of their actions.

The Control Room Operator should carry out the following actions:

Confirm that all water drainage valves are shut and wastewater pumps are shut down.
Inform the Shift Charge Engineer of the leak
Locate and refer to the relevant Material Safety Data Sheet (MSDS)
Mobilise the Emergency Response Team if required
Sound the station evacuation alarm and or inform staff of any hazards using the PA
system if required
Summon the emergency services as required

Delegate staff to meet at site entrance and act as guide for the emergency services if
necessary
Assume roll of INCIDENT CONTROLLER if necessary and don bib for easy
recognition.
8. Field Operators
On hearing the evacuation alarm the Field Operators are to contact the control room for
instructions.

9. Incident Controller (Shift Charge Engineer or delegate)

Don the Incident Controllers Bib for ease of recognition.


Establish communications with the Control Room.
Inform General Manager and Operations Manager
Liaise with emergency services
Authorise "All Clear" on instruction from emergency services.

10. Security Staff


On being informed of an incident by the control room

Stop any traffic or persons from entering site


Maintain clear access to site for the Emergency Services
Contact security staff at the project gate and ensure access is allowed for emergency
services
If site evacuation alarm has been sounded, ensure all staff, contractors and visitors
are accounted for
Ensure no one leaves site until released by the Incident Controller.
Perform duties of ROLL OFFICER

11. First Aiders


First Aiders that may be called to treat casualties at the incident, either from their place of work
or from the muster point during a site evacuation should ensure that they establish and identify
a safe route to and from the incident. They should always ensure that they have their PPE
with them to enable access to site if required.

12. All Other Staff


If the site evacuation alarm has been sounded, personnel without specific duties during the
incident shall report to an assembly point and ensure they are accounted for. Staff will not
leave the assembly point until authorised to do so by the Incident Controller.
Staff meeting visitors on site shall ensure that they are familiar with the emergency procedures
and will be responsible for their Health, Safety and Welfare whilst on site. On hearing the
evacuation alarm, staff with visitors shall guide them in an orderly fashion by a safe route to
the assembly point, where they will remain until accounted for and the "all clear" is given.

Any Permits for Work must be surrendered and reissued before restarting work.

Appendix 5 Actions in event of major first aid incident

1. Definitions
A 'First Aid Incident' in the context of this document is defined as:

'First Aid' only - maintain life and mitigate injury until Fire Team or ambulance service arrives

'First Aid' will only be carried out by accredited First Aiders & Basic First Aiders (within their
competence) until accredited First Aider arrive.

2. Action on receipt of a call for First Aid


The person receiving the call should ascertain and log the following

Time of call
Name of Person making the call
Location of the casualty
Nature and severity of injuries
Cause of injuries if known

The person receiving the call should carry out the following actions:

Using the PA system put out a general call for any First Aider to contact the control
room.
When contacted by First Aider, inform them of the nature and severity of injuries and
direct them to the casualty’s location
Inform the Shift Charge Engineer of the details of the incident
Summon an ambulance if requested by the First Aider on scene

4. First Aider
On hearing the PA request for First Aider they should:

Contact the Control Room for directions including safe route to the casualties if during
site Evacuation.
At incident, if safe to do so, treat casualties
Liaise with ambulance crew during casualty evacuation
After incident, ensure area is clear of used equipment etc.
Replenish any used equipment.
Fill in first aid referral form, accident book and treatment slip and Intelex reporting
software

They should always ensure that they have their PPE with them to enable access to site if
required.
Az Zour South also has a medical room and has agreed to assist in the event of a medical
emergency.
